摘要 目的总结青年人颈椎MRI表现,分析颈椎间盘Pfirrmann分级与低头时间、体质指数(BMI)、生理曲度、椎间盘形态及性别的关系。方法对66例在我院行颈椎MRI检查的青年人,依据每日总共低头时间分为<1 h组、1~3 h组和>3 h组。评估颈椎生理曲度、椎间盘突出/膨出及终板改变。根据Pfirrmann分级标准对颈椎间盘T2信号进行分级。分析不同低头时间、BMI、生理曲度、椎间盘形态、性别与颈椎间盘Pfirrmann分级有无差异及相关性。结果 66例患者共评价396个椎间盘。不同低头时间、BMI、生理曲度、性别的椎间盘Pfirrmann分级差别有统计学意义(P<0.01);不同椎间盘形态的Pfirrmann分级的差别无统计学意义(P>0.05)。颈椎间盘Pfirrmann分级与低头时间、BMI、性别显著相关,与生理曲度、椎间盘形态无明显相关性。结论青年人颈椎MRI椎间盘信号减低和生理曲度异常的发生率较高,颈椎间盘Pfirrmann分级与生活习惯密切相关。 Objective To identify the MRI features of the cervical spine of youth and to analyze the relationships between cervical intervertebral disc Pfirrmann grading,the length of time the head is kept down,body mass index(BMI),intervertebral disc morphology and gender.Methods A total of 66 young people who had under gone cervical MRI examinations in our hospital were divided into three groups:1 hour,1-3 hours,and 3 hours according to the total length of time the head is kept down per day.Physiological curvature,intervertebral disc herniation and endplate changes were evaluated.T2 signals of the cervical intervertebral disc were classificated according to Pfirrmann grading standard.The difference and correlation between intervertebral disc Pfirrmann classification and the length of time the head is kept down,BMI,physiological curvature,intervertebral disc morphology and gender were analyzed.Results A total of 396 intervertebral discs were evaluated for 66 patients.There was statistically significant difference in intervertebral disc Pfirrmann classification according to the length of time the head is kept down,BMI,physiological curvature and gender(P = 0.000),but no correlation with intervertebral disc morphology.Intervertebral disc Pfirrmann classification was correlated with the length of time the head is kept down,BMI and gender(P = 0.000),but not with physiological curvature or intervertebral disc morphology.Conclusion The incidence of signal decrease and abnormal physiological curvature of the intervertebral disc is high in cervical MRI of young people.Pfirrmann classification of the cervical intervertebral disc is closely related to habits of life.
