摘要
目的探讨非瓣膜性房颤中医辨证分型与CHA2DS2-VASc评分的相关性,以及初步分析房颤患者中医辨证分型与其一般情况、合并症、心脏超声相关指标的相关性。方法随机抽取房颤患者133例,收集患者的一般情况、合并症、心脏超声相关指标,根据临床症状分辨中医证候,其中非瓣膜性房颤123例,以CHA2DS2-VASc评分评定血栓风险评分。结果 CHA2DS2-VASc评分1~2分组心血不足证型20例,5~9分组水饮凌心证型25例,均明显高于其他分值组,差异有统计学意义(P<0.05)。水饮凌心证型组射血分数(EF)明显低于心血不足证型组和瘀阻心脉证型组,左房内径明显高于心血不足证型组和瘀阻心脉证型组,左室舒张末内径明显高于心血不足证型组,差异有统计学意义(P<0.05)。不同中医证型组中合并冠心病的比例为:瘀阻心脉组77%,水饮凌心组48%,心血不足组23%;不同中医证型组中合并心衰的比例为:水饮凌心组94%,心血不足组30%,瘀阻心脉组23%。结论房颤水饮凌心患者血栓形成风险较高,患者多合并心力衰竭、左心房发生扩大;房颤瘀阻心脉患者左心房内径、左心室舒张末内径正常,患者多合并冠心病;房颤心血不足证患者左心房内径、左心室舒张末内径、左心射血分数多正常。
Objective To explore the correlation between dialectical type in traditional Chinese medicine and CHA2DS2-VASc score of nonvalvular atrial fibrillation, and to preliminarily analyze the correlation between dialectical type of atrial fibrillation in traditional Chinese medicine and general data, complications, and cardiac ultrasound-related indicators. Methods A total of 133 cases of patients with atrial fibrillation were selected randomly, the general data, complications ,and cardiac ultrasound-related indicators were collected, and the dialectical type in traditional Chinese medicine was distinguished according to clinical symptoms. In these cases, there were 123 patients with nonvalvular atrial fibrillation. Thrombosis risk score was assessed by CHA2DS2-VASc. Results There were 20 cases of Xinxuebuzu genre in CHA2DS2-VASc score 1- 2 group, and 25 cases of Shuiyinlingxin genre in CHA2DS2-VASc score 5 - 9 group, which were significantly higher than those of the other score groups （ P 〈 0.05 ）. The EF of Shuiyinlingxin genre group was lower than that of Xinxuebuzu genre group and Yuzuxinmai genre group （ P 〈 0.05 ）, while the LAD was higher （ P 〈 0.05 ） ; the LVD in Shuiyinlingxin genre group was higher than Xinxuebuzu genre group （ P 〈 0.05 ）. The occurrence rates of complication of coronary heart disease were 77%, 48% and 23% in Yuzuxinmai genre group, Shuiyinlingxin genre group and Xinxuebuzu genre group; the occurrence rates of complication of heart failre were 94% ,30% and 23% in Yuzuxinmai genre group, Shuiyinlingxin genre group and Xinxuebuzu genre group. Conclusion The patients with Shuiyinlingxin genre of atrial fibrillation have a higher rate of thrombosis, which is complicated with heart failure and left atrium expanded mostly; Yuzuxinmal genre of atrial fibrillation is complicated with coronary heart disease mostly, which have normal levels of LAD and LVD; Xinxuebuzu genre of atrial fibrillation have normal levels of LAD,LVD and LVEF.
