摘要 目的:探讨不同氨基酸营养支持方案对早产儿生长发育及代谢的影响。方法:选取2014年6月~2016年9月我院妇产科出生的早产儿74例,随机分为观察组和对照组,分别行初始大剂量方案和快速增加方案,对比2组患儿生长发育指标、代谢指标、喂养不耐受率及并发症发生率。结果:观察组患儿体质量下降幅度降低,体质量、头围、身长增加速度增快,恢复出生体质量时间、静脉营养支持时间缩短(P<0.05);出生后14 d,观察组患儿血清Pre-ALB、IGF-1、瘦素、MOT、VIP水平较对照组升高(P<0.05),Cr、BUN、DBiL、TBA、CO_2CP水平与对照组差异无统计学意义(P>0.05);观察组喂养不耐受率较对照组降低(2.7%vs.21.6%)(P<0.05);2组患者并发症发生率差异无统计学意义(P>0.05)。结论:早期给予大剂量氨基酸营养支持有利于早产儿的生长发育,促进胃肠道功能成熟,减少喂养不耐受率,且对机体代谢影响小,安全性良好。 Objective : To investigate the effects of different amino acid nutritional support regimens on the growth, development and metabolism of preterm infants.Methods:Seventy-four preterm infants delivered in our department between June :2014 and September 2016 were included, and randomized into observational group and control group according to the difference of amino acid nutrition support program.Infants in the observational group received initial high-dose amino acids nutritional support, and those in the control group were treated with rapid increase regimen amino acid nutritional support.Then the two groups were compared concerning the growth and development indicators, metabolic indicators, feeding intolerance rate and incidences of complications. Results : The declining degree of body weight was significant in the observational group, with marked increase rate of body mass, head circumference and body length, and the recovery time of birth weight and the time of vein nutrition support were shortened( P〈0.05 ).Levels of Pre-ALB, IGF-1, leptin, motilin (MOT) and vasoactive intestinal peptide( VIP ) were higher in the observational group (P〈0.05), yet the difference was not significant between groups regarding the levels of creatine (Cr) , blood urea nitrogen ( BUN ) , direct bilirubin (DbiL) , total bile acid ( TBA ) and carban dioxide-combining power (CO2CP) (P〉0.05).Infants in the observational group had higher feeding intolerance rate than those in the control group( P〈0.05 ) , yet the difference was insignificant in complications between groups(P〉0.05).Conelusion:Initiai high-dose regimen of amino acids can benefit to the growth and development of preterm infants,promote gastrointestinal function maturation,reduce the feeding intolerance rate,and produce few effect on the body metabolism, suggesting that this regimen can be good therapeutic effects.
