摘要
目的分析皮肤点刺试验级别与舌下含服粉尘螨滴剂治疗中-重度变应性鼻炎疗效的关系。方法收集2012年01月-2016年12月来南京医科大学附属南京第一医院就诊的117例诊断明确为中-重度变应性鼻炎的患者为研究对象,均以表转化粉尘螨滴剂进行12个月的SLIT,期间联合对症药物。按就诊时皮肤点刺试验(SPT)级别分为4组:第1组(SPT为1个+,28例),第2组(SPT为++,28例);第3组(SPT为+++,30例),第4组(SPT为++++,31例),评估不同SPT组别患者鼻部总症状评分(TNSS)、总药物评分(TMS)及体征评分的变化。结果 SLIT 3个月、6个月、12个月后,不同点刺级别组的AR患者的TNSS、TMS以及体征评分较治疗前均显著改善(均P<0.05=;TNSS、TMS以及体征评分组间均差异无统计学意义(均P>0.05)。结论采用标准化粉尘螨滴剂对不同点刺级别的AR患者进行12个月的SLIT均有疗效;不同点刺级别患者间取得的相近的疗效。
Objective To analyze the effect of sublingual immunotherapy with Dermatophagoides farinae dropsin on moderate to severe allergic rhinitis patients with different skin prick test（SPT）level. Method he data of 117 patients with moderate to severe allergic rhinitis whose allergen was dust mites treated in Nanjing First Hospital of Nanjing Medical from January 2011 to December 2016 were retrospectively analyzed. All patients were treated with Dermatophagoides farinae drops and combined with symptomatic treatment. The patients were divided into 4 groups according to skin prick test（SPT）level, group 1 （SPT：＋, 28 cases）; group 2 （SPT：＋＋, 28 cases）;group 3 （SPT：＋＋＋, 30 eases）;group 4 （SPT：＋＋＋＋, 31 cases）. To evaluate the changes of nasal total symptom score （TNSS）, total drug score （TMS） and signs score in patients with different SPT groups. Result The total nasal symptom scores（TNSS）, total medicine scores （TMS） and signs scores had continuously improved significantly after SLIT for 3 months ,6 months and 12 months （P〈0.05）. There were no significant differences in all groups for TNSS ,TMS and sign scores（P〉0.05 ）. Conclusion SLIT with Dermatophagoides farinae drops for 12 months is effective in moderate to severe allergic rhinitis patients with different SPT level. And equivalent efficacy can be achieved for different SPT level.
