蚌埠市部分社区中老年居民膳食模式与骨密度关系研究 被引量:6

DIETARY PATTERNS AMONG MIDDLE-AGED AND ELDERLY PEOPLE AND ITS RELATION TO BONE MINERAL DENSITY IN BENGBU CITY

摘要 目的了解蚌埠市社区居民的骨密度现状,并分析膳食模式与骨密度的关系。方法采用分层整群随机抽样的方法,抽取蚌埠市6个社区的860名45岁以上的中老年居民进行调查,检测人群的超声骨密度值,采用因子分析法建立膳食模式,通过协方差分析比较不同膳食模式组间的骨SOS修正均数。多因素Logistic回归分析骨质疏松的膳食影响因素。结果 (1)860名居民中,骨质疏松症的患病率为21.6%,骨SOS均值为(1502.68±27.03)m/s。不同性别、年龄、体重、BMI、腰臀比、绝经年龄和体力活动水平人群的骨质疏松患病率差异有统计学意义(P<0.05)。(2)通过因子分析得到了4种膳食模式,主食蔬菜型模式、动物型模式、水果零食型模式和含钙食物型模式。在多因素非条件logistic回归中,调整了性别、年龄、文化程度、是否绝经、体力活动和体质指数后,显示"动物型"膳食模式和"含钙食物型"膳食模式对骨质疏松症起保护作用,OR值分别为0.942(0.454~1.451),0.612(0.362~0.947),均P<0.05。结论膳食模式与骨质疏松密切相关,应注意全面而合理的膳食来预防骨质疏松的发生。 Objective To characterize the status of bone density of community residents, and analyze the association between dietary patterns and bone mineral density (BMD). Methods A stratified random sampling was used. Supersonic bone mineral density was measured with CM-200 ultrasound device. Factor analysis was applied to determine major dietary patterns. Logistic regression was used to examine the association between dietary patterns and BMD. Results (1)A total of 860 community residents aged ≥45 years old from 6 communities of Bengbu city were chosen as the subjects. The mean speed of sound (SOS) in bone was (1502.68±27.03) m/s and the prevalence of osteoporosis was 21.6%. The prevalence of osteoporosis was different among various groups of different sex, age, weight, body mass index, waist to hip ratio, menopause age and physical activity. All differences were significant (P〈0.05). (2) There were four major dietary patterns ("staple food" pattern, "animal" pattern, "fruit snacks" pattern and "calcium" pattern). After adjusted for sex, age, educational level, menopause, physical activity and body mass index, the multivariate logistic regression showed that "animal" pattern and "calcium" pattern could reduce the risk of osteoporosis. Compared with the lowest quartile (Q1), the person in the highest quartile for animal food and calcium food consumption (Q4) had a lower risk of osteoporosis, OR values were 0.942 (0.454-1.451) and 0.612 (0.362-0.947)(all P 〈0.05). Conclusion Dietary patterns are related with osteoporosis. We should follow a rational dietary pattern to prevent against osteoporosis.
