血清C-反应蛋白和降钙素原检测对脑出血术后伴颅内感染的诊断及预后评估价值 被引量:9

Value of serum C reactive protein and procalcitonin detection in diagnosis and prognosis evaluation of intracranial infection of postoperative cerebral hemorrhage

摘要 目的探讨血清C-反应蛋白(CRP)和降钙素原(PCT)检测对脑出血术后伴颅内感染的诊断及预后评估价值。方法将185例行开颅血肿清除术的脑出血患者根据术后是否发生颅内感染分为感染组(60例)和无感染组(125例);留取患者的脑脊液标本和血液标本,分别进行病原菌培养和血清CRP、PCT水平检测。结果 60例感染标本检出病原菌72株,革兰阴性菌、革兰阳性菌及其他病原菌分别占30.56%、48.61%和20.83%;感染组血清CRP、PCT水平均明显高于无感染组;细菌性感染组血清PCT水平明显高于非细菌性感染组;中、重度感染者血清CRP、PCT水平均明显高于轻度感染者,且重度感染者比中度感染者更高。抗菌药治疗前后,存活组的血清CRP、PCT水平均明显低于死亡组,上述差异均有统计学意义(P<0.05)。结论血清CRP、PCT水平测定可用于脑出血术后伴颅内感染的诊断及预后评估。 Objective To investigate the application value of serum C reactive protein( CRP) and procalcitonin( PCT) detection in the diagnosis and prognosis evaluation of intracranial infection of postoperative cerebral hemorrhage. Methods 185 patients with cerebral hemorrhage who undergoing the evacuation of hematoma through craniotomy were divided into infection group( n =60) and non infection group( n = 125) according to the occurrence of intracranial infection after operation. The cerebrospinal fluid( CSF) samples and blood samples of all the patients were collected. The pathogenic bacteria in CSF were cultured and the levels of CRP and PCT in serum were detected. Results 72 strains of pathogenic bacteria were isolated from the 60 infection specimens. Among which,gram negative bacteria,gram positive bacteria and other pathogens accounted for 30. 56%,48. 61% and20. 83%,respectively. Compared with the non infection group,the levels of serum CRP and PCT of the infection group were significantly higher. The level of serum PCT in patients with bacterial infection was obviously higher than that of patients with non bacterial infection. Compared with the mild infection patients,the levels of serum CRP and PCT of the moderate and severe infection patients were both significantly higher. Moreover,the levels of serum CRP and PCT of the severe infection patients were obviously higher than that of the moderate patients. Before and after the treatment of antibacterials,the levels of serum CRP and PCT of the survival group were obviously lower than those of the death group,with all the differences above statistically significant( P〈0. 05). Conclusion The serum levels of CRP and PCT could be used for the diagnosis and prognosis of intracranial infection after intracerebral hemorrhage.
