细胞免疫荧光法在视神经脊髓炎患者血清及脑脊液中水通道蛋白-4抗体检测中的运用价值

摘要 目的探讨细胞免疫荧光法在视神经脊髓炎患者血清及脑脊液中水通道蛋白-4抗体(APQ-4Ab)检测中的运用价值。方法选择在2013年6月至2017年4月在某院诊断为视神经脊髓炎患者58例(NMO组)和49例多发性硬化患者(MS组),以及体检健康者45例作为研究对象,收集相关人群的血清和脑脊液。分别采用细胞免疫荧光法(CBA)、乳胶免疫增强比浊法(PETIA)和酶联免疫法(ELISA)检测APQ-4Ab,并分析相关指标。结果3种方法分别在3组人群的血清和脑脊液中APQ-4Ab检测中差异均有统计学意义(P<0.05)。但是在NMO组的脑脊液和血清中,CBA法对于APQ-4Ab检测的阳性率均高于PETIA和ELISA(P<0.05),而在MS组,3者差异不大。CBA法的敏感性为83.1%,高于PETIA的62.4%和ELISA的57.6%(P<0.05),3种检测方法的特异性都可达到90%以上,但差异无统计学意义(P>0.05)。结论细胞免疫荧光法在视神经脊髓炎患者血清及脑脊液中APQ-4Ab检测中具有较高的准确性,且敏感性和特异性俱佳。 Objective To explore the application value of cell-based assay method in the water channel protein-4 antibody detection in serums and cerebrospinal fluids of patients with optic spinal meningitis.Me thods From June2013 to April 2017,58 cases of patients diagnosed with neuro-optic myelitis(NMO),49 cases of patients with multiple sclerosis(MS) and 45 healthy people were collected as the research subjects.Their serum and cerebrospinal fluids were collected by a hospital.Cell-based assay(CBA),particle-enhanced turbidimetric immunoassay(PETIA) and diagnostic kit for Ig A to helicobacter pylori(ELISA) were adopted to detect APQ-4 Ab and relevant indexes were analyzed.Re s ults The 3 methods showed differences in the detection in serums and cerebrospinal fluids among patients in three groups and APQ-4 Ab and the differences were statistically significant( P<0.05).In the detection of cerebrospinal fluids and serums of the NMO group,the APQ-4 Ab positive rate of CBA were higher than those of PETIA and ELISA(P <0.05).In MS groups,the differences were not significant.The sensitive rate of CBA was 83.1%,higher than 62.4% of PETIA and 57.6% of ELISA with(P <0.05).The specificity of the three methods were higher than90% but the difference was not statistically significant( P >0.05).Conclus ion Cell-based assay method enjoyed high accuracy rate in the detection of water channel protein-4 antibody in serums and cerebrospinal fluids of patients with optic spinal meningitis,with high sensitivity and specificity.
