摘要
目的:观察益气养血祛瘀泄浊法对5/6(A/I)慢性肾衰竭(CRF)模型大鼠蛋白尿及残余肾功能的保护作用。方法:采用经典的5/6(A/I)肾切除CRF模型法制作CRF动物模型,随机分为正常组(A组)、模型组(B组)、西药组(氯沙坦钾C组)、中药组(肾衰Ⅱ号方D组),给予相应干预措施,疗程60 d。于治疗前后分别检测24 h尿蛋白定量(24 h Up)、肌酐(Scr)、尿素氮(BUN)、尿酸(UA)、收缩压(SMP)、舒张压(DMP)等指标,计算内生肌酐清除率(Ccr)。结果:造模后,与A组相比,B组、C组、D组24 h Up升高;Scr、BUN、UA、SMP、DMP升高(P<0.05),Ccr、Hb、RBC降低(P<0.05),表示造模成功。干预后,D组大鼠精神状态、毛发色泽、活动度等明显优于其他几组;与B组比较,C组、D组24 h Up降低(P<0.05);两组Scr、BUN降低(P<0.05),Ccr升高(P<0.05);C组UA显著降低(P<0.01),D组Hb、RBC水平升高。结论:肾衰Ⅱ号方可保护残余肾功能,降低血清肌酐、尿素氮、尿酸水平,提高肌酐清除率,改善贫血,延缓肾衰竭;在降低尿蛋白排泄方面与西药组疗效相当,但肾衰Ⅱ号方在改善大鼠精神、活动状态等生活质量方面优于西药组。
Objective：To observe the effects of Tomifying qi and blood and remove stasis drain turbidity method for proteinuria in chronic renal failure rats induced by 5/6 Ablation/Infarction. Methods：Rats,which were made into chronic renal failure（CRF） an imal models by means of 5/6 Ablation/Infarction,were divided randomly into normal group,model group,western medicine（Losartan Potassium） group and Chinese medicine（Renal Failure Ⅱ Decotion） group. Each group was given relevant intervention treatment by administration for 60 days. Before and after treatment,test 24-hour urinary protein quantitative（24 h Up）,serum creatinine（ Scr）,blood urea nitrogen（ BUN）,uric acid（ UA）,the tail artery systolic pressure（ SBP）,diastolic blood pressure（ DBP）,hemoglobin（ Hb）,red blood cell（ RBC）,measure creatinine clearance rate（ Ccr）. Results：After being modeled,compared with Group A,24 h Up in Group B,C and D increased（ P 〈 0. 05）,the leve l of Scr,BUN,UA,SMP,DMP increased（ P 〈 0. 05）,Ccr,Hb,RBC decreased（ P 〈 0. 05）,demonstrating the successful modeling. After intervention treatment,the mental state,hair color,activity and other aspects of group D were significantly better than other groups. In Group C and D compared w ith Group B,the leve l of Scr,BUNdecreased（ P 〈 0. 05）,Ccr increased（ P 〈 0. 05）,the level of UA in group C was significantly increased（ P 〈 0. 01）; the level of Hb and RBC in group D was increased. Conclusion：Renal Failure Ⅱ Decotion can protect residual renal function,decreased the leve l of Scr,BUN,UA,increased Ccr,improve anemia,delay the renal failure,and the role of reducing urinary protein excretion was same as the western medicine group. However,the role of improving the quality of life in rats,for example,mental state,activity,was better than western medicine group. In addition.
