冻干羊膜重建腱鞘缺损对肌腱愈合的早期生物力学影响 被引量:5

摘要 目的采用冻干羊膜重建腱鞘缺损,探讨其对肌腱愈合及肌腱粘连的早期生物力学的影响。方法选取来亨鸡40只,随机分为实验组及对照组,每组20只。选用来亨鸡双足第Ⅲ足趾肌腱及腱鞘损伤模型:实验组缝合趾屈深肌腱断端后用两层冻干羊膜重建缺损的腱鞘;对照组直接缝合趾屈深肌腱断端,不进行腱鞘的修复。术后1、2、3、4周取材行生物力学检测,观察2组肌腱最大拉伸断裂强度及拉断肌腱粘连带功耗。结果实验动物均存活至实验完成,切口愈合好。2组均在2周时最大拉伸断裂强度下降,3周和4周时逐渐升高,术后1、2、3、4周各个时点实验组肌腱最大拉伸断裂强度均大于对照组,差异有统计学意义(P<0.05);2组拉断肌腱粘连带功耗在1、2、3、4周均呈逐渐升高趋势,实验组在1、2、3、4周每个时点拉断肌腱粘连带功耗均低于对照组,差异有统计学意义(P<0.05)。结论冻干羊膜重建腱鞘缺损能阻止肌腱外源性愈合,促进内源性愈合,提高了肌腱愈合质量,为早期主动功能训练提供了必要条件。 Objective To explore the biomechanical characteristics of tendon healing and tendon adhesion after the freeze-dried amniotic membrane was used to reconstruct tendon sheath defect.Methods Forty healthy male leghorn chickens were used in this study.These animal models were randomly divided into two groups,the therapy group and the control group,with twenty chickens in each group.The model of tendon injury and tendon sheath defects was established in the third toes of chicken.After repairation of the flexor digitorum profundus tendon,a double freeze-dried amniotic membrane was used to reconstruct the tendon sheath defect of model in the therapy group,while the tendon sheath defect was not reconstructed in the control group.Five chickens were randomly selected from each group at the 1 st,2 nd,3 rd and4 th week for biomechanical analysis.Biomechanical parameters,including the tensile strength of rupture and the tensile adhesion strength of the rupture zone were measured at each time point.Results All experimental animals survived until the experiment was finished,and all incision healed well.The tensile strength of rupture decreased after the first two weeks,and increased gradually at the 3 rd and 4 th week.At the 1 st,2 nd,3 rd and 4 th week after the operation,the tensile strength of rupture in the therapy group was significantly higher than that in the control group(P<0.05).The power consumption of the 2 groups was increased gradually at 1,2,and3,4 weeks.In the experimental group at 1,2,3,4 weeks,the power consumption of the tendon adhesion zone was lower than that of the control group.The differences are statistically significant(P<0.05).Conclusion Reconstruction of tendon sheath defects by freeze dried amniotic membrane can prevent tendon from exogenous healing,promote endogenous healing,and improve the quality of tendon healing,which provides a necessary condition for early active functional training.
