摘要
目的研究Th1/Th2细胞相关因子在非酒精性脂肪肝(NAFLD)合并幽门螺杆菌(HP)感染大鼠中的表达。方法采用对照试验研究方法,将80只雄性SD大鼠随机分为对照组、NAFLD组、HP组及NAFLD+HP组。构建NAFLD模型、HP模型和NAFLD+HP模型,饲养满12周。之后处死所有大鼠,四组大鼠分别抽取外周血,对超敏C反应蛋白(hs-CRP)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、三酰甘油(TG)、肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)、白细胞介素-10(IL-10)、白细胞介素-22(IL-22)进行检测。NAFLD+HP组大鼠抗HP治疗后,外周血再行上述检测。用光学显微镜进行四组胃黏膜组织的HP判定,用免疫组化染色法检测各组肝组织中TNF-α、IFN-γ、IL-10、IL-22水平及组织中分布。结果 ALT、AST、CRP、TG、TNF-α、IFN-γ和IL-22水平,NAFLD+HP组、HP组和NAFLD组与对照组对比明显升高,NAFLD+HP组最高,差异有统计学意义(P<0.05);抗HP治疗后NAFLD+HP组CRP、ALT、AST、TNF-α、IFN-γ、IL-22水平比治疗前下降,IL-10水平明显比治疗前上升,差异有统计学意义(P<0.05);TG水平治疗前后对比差异无统计学意义(P>0.05);光学显微镜下显示,对照组的胃粘膜完整,上皮固有层内的腺体排列紧凑,有少量的淋巴细胞,NAFLD+HP组大鼠固有层可见较多的淋巴细胞、中性粒细胞和嗜酸性粒细胞,胃部炎症严重,而HP组和NAFLD组比NAFLD+HP组相对较轻。肝组织中的TNF-α、IFN-γ、IL-10和IL-22的免疫组化检测结果显示,4组细胞因子的表达在NAFLD+HP组、HP组和NAFLD组与对照组比较均高。结论NAFLD与HP有相关性,且HP感染会通过Th1/Th2细胞因子的失衡促进及加重NAFLD,抗HP治疗不但有利于消除HP,而且对治疗NAFLD也有一定的效果。
Objective To investigate the expression of Th1/Th2 cell-associated factors in nonalcoholic fatty liver disease (NAFLD) and with Helicobacter pylori (HP) infection in rats. Methods 80 male SD rats were randomly divided into control group,NAFLD group,HP group and NAFLD+HP group. NAFLD model,HP model and NAFLD+HP model were established for 12 weeks. After all ,the rats were killed;peripheral blood levels of hypersensitive C-reactive protein (hs-CRP),alanine aminotransferase(ALT),aspartate transaminase(AST),triacylglycerol(TG),tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ),IL-10 and IL-22 were detected. The HP of gastric mucosa was determined by optical microscope. The levels of TNF-α,IFN-γ,IL-10 and IL-22 in liver tissue were detected by immunohistochemical staining. Results ALT,AST,CRP,TNF-α,IFN-γ,and IL-22 levels in NAFLD+HP group,HP group and NAFLD group were significantly higher than those in control group and NAFLD+HP group(P<0.05). The levels of CRP,ALT,AST,TNF-α, IFN-γ and IL-22 in NAFLD+HP group were significantly lower than those before treatment ,and the levels of IL-10 were significantly higher than those before treatment(P<0.05). There was no significant difference between the two groups before and after treatment (P>0.05). Under the optical microscope ,the gastric mucosa of the control group was intact and the glands in the lamina propria were compact,with a small amount of lymphocytes,and the lymph nodes,neutrophils and eosinophils were observed in the lamina propria of the NAFLD+HP group. Granulocyte and stomach inflammation of HPgroup and NAFLD group were relatively lighter than those of NAFLD+HP. The expression of TNF-α,IFN-γ,IL-10 and IL-22 in liver tissue showed that the expression of cytokines in NAFLD+HP group ,HP group and NAFLD group were higher than those in control group.
