摘要 目的探讨外周灌注指数对重症患者脉搏血氧饱和度测量准确性的影响。方法对200例ICU重症患者通过监护仪脉搏血氧饱和度探头测量外周灌注指数和脉搏血氧饱和度,同时抽取动脉血气测量动脉血氧饱和度;根据外周灌注指数分为灌注良好组和灌注不足组,比较两组脉搏血氧饱和度及动脉血氧饱和度及测量准确性(SpO_2与SaO_2的差异在±3%内)。结果经外周灌注指数测定,灌注良好103例,不足97例。灌注良好组和灌注不足组脉搏血氧饱和度测量准确率分别为97.1%、77.3%,两组比较,差异有统计学意义(P<0.01)。结论外周灌注指数对脉搏血氧饱和度测量的准确性有影响;外周灌注不足时,脉搏血氧饱和度测量准确性下降,建议可同时结合动脉血气血氧饱和度动态观察患者氧合变化。 Objective To explore the impact of peripheral perfusion index (PI) on accuracy of pulse oximetry in intensive care pa- tients. Methods PI and pulse oximeter oxygen saturation (SpO2) were measured using pulse-oximeter sensors of the electrocardio- gram (ECG) recording and monitoring system in 200 intensive care patients, and arterial blood gas was examined for arterial oxy- gen saturation (SaO2). According to the PI, patients were divided into a good perfusion group (PI〉 4) and a poor perfusion group(PI≤ 4), then SpO2 , SaO2 and measuring accuracy (difference between SpO2 and SaO2 less than ± 3%) were compared between the two groups. Results One hundred and three patients were in good perfusion group and 97 in poor perfusion group. The measuring accuracy was 97.1% in good perfusion group and 77.3 % in poor perfusion group, with significant difference between them (P〈0.01). Conclusion PI can affect accuracy of pulse oximetry measurement; and the accuracy of pulse oximetry measure- ment was decreased when PI was≤1.4. SaO2 was recommended as a supplementary parameter to reflect patients' oxygenation changes.
