摘要
介绍周德安教授应用针刺配合中药治疗感音神经性耳聋的临床经验。周德安重视脏腑及经络辨证,主张针药并用。针刺治疗方面急性期肝胆火旺者,治以清泻肝胆、通利耳窍,创立针刺穴组为通耳方;慢性期属肝肾不足者,治以补益肝肾、镇静安神,创立取穴组为聪耳方。中药内服方面急性期治疗以疏肝活血为主,慢性期治疗以健脾补肾为主。认为临床中不同时期均可能存在虚实的变化,要根据患者具体的舌脉进行辨证,掌握不同时期患者虚实之间的变化及时调整治疗方案以获得更好的治疗效果。
This article introduces Professor ZHOU De＇ an＇ s clinical experience in treating sensorineural hearing loss（ SNHL） by combining acupuncture and Chinese herbs. ZHOU De＇ an emphasizes on Zang-fu and channel syndrome differentiation and advocates combined use of acupuncture and Chinese herbs. In terms of acupuncture treatment,for acute vigorous fire in liver and gallbladder,he clears liver heat and drains gallbladder,dredges ear orifices and establishes an acupuncture point group as Tong＇er Fang（ 通耳方）; for chronic liver and kidney deficiency,he nourishes liver and kidney,tranquilizes and allays excitement,and establishes another acupuncture point group as Cong＇er Fang（ 聪耳方）. In terms of Chinese herbs,soothing liver and invigorating blood is the key during acute stage,while fortifying spleen and supplementing kidney is the key during chronic stage. He thinks that deficiency and excess may exist during different clinical periods,so physicians should differentiate the syndrome according to the specific tongue and pulse of the patient,to grasp the changes between excess and deficiency in different periods,and adjust the treatment options to obtain better therapeutic effect.
