摘要
目的:探讨快通道麻醉对法洛四联症患儿手术的影响。方法:回顾性分析2013-2014年收治的564名行手术治疗的法洛四联症患儿的临床资料,其中2013年279例患儿作为A组,2014年282例患儿为B组。比较两组手术时间、CBP时间、主动脉阻断时间、术毕患儿拔管时间、ICU停留时间、住院时间、再插管率、围术期用血量、术后并发症等。结果:A组术前ASA分级高于B组(P<0.01)。A组手术时间、CPB时间、升主阻断时间少于B组(P<0.05),术后带管时间长于B组(P<0.05)。A组快通道实施例数为177例(63.4%),少于B组的210例(74.5%,P<0.05)。A组使用浓缩红细胞量、新鲜冰冻血浆量多于B组(P<0.05)。结论:快通道麻醉技术可以安全应用于法洛四联症患儿,并不增加术后再插管、术后再次手术、术后死亡等并发症的发生。
Objective：To investigate the effect of fast track anesthesia in children with tetralogy of Fallot. Methods：The clinical data of 564 children with tetralogy of Fallot,who were treated between 2013 and 2014,were retrospectively analyzed. A total of 279 children treated in 2013 were included in group A, and 282 children treated in 2014 were included in group B. The operative time,CBP time,aortic cross-clamping time,postoperative extubation time, ICU stay, hospital stay, reintubation rate, perioperative blood volume, and postoperative complications were compared between the two groups.Results：The postoperative ASA grade in group A was higher than that in group B （P〈0.01）. The operative time,CBP time,and aortic cross-clamping time in group A were shorter than those in group B （ P〈0.05） ,whereas the postoperative intubation time was shorter than that in group B （P〈0.05）. The number of fast track cases in group A was 177 （63.4%）,which was less than 210 in group B （74.5%,P〈0.05）. The amount of concentrated erythrocytes and fresh frozen plasma used in group A was more than that in group B （ P〈0.05） . Conclusion：Fast track anesthesia can be safely used in childrens with tetralogy of Fallot is safe,without increasing the incidence of complications such as postoperative reintubation,postoperative reoperation and postoperative death.
