摘要
目的观察通过微创手术切除腺样体以治疗儿童阻塞性睡眠呼吸暂停低通气综合症(OSAHS)并发中耳炎的临床效果.为临床治疗提供参考。方法回顾性分析2010年6月至2015年6月110例OSAHS并发中耳炎患儿的临床资料。结果与对照组比较,三个年龄段的观察组患者的术后鼓室导抗图恢复时间均明显减少(P〈0.05);与治疗前比较,两组患者的特异性生活质量调查(OSA-18)总评分、呼吸暂停低通气指数(AHI)均明显降低,最低血氧饱和度(LSaO2)均明显升高,且观察组患者上述三项指标改变更明显(P〈0.05);观察组患儿中耳积液时间为(6.1±1.4)d,明显少于对照组的(9.9±2.6)d,且观察组患儿术后感染例数明显少于对照组(P〈0.05);在气导阈值比较中,观察组与对照组患者治疗后的阈值均明显低于治疗前,且治疗后观察组患者的气导阈值明显低于对照组(P〈0.05);观察组临床总有效率为94.5%,明显高于对照组76.4%(P〈0.05)。结论采用低温等离子刀切除腺样体治疗儿童OSAHS并发中耳炎临床效果显著,改善儿童听力情况更好,且观察组患儿特异性生活质量调查和呼吸指数改善均好于对照组,改善中耳炎效果更明显,值得推广应用。
Objective To study the clinical effect of minimally invasive surgery in the treatment of obstructive sleep apnea hypopnea syndrome complicated with otitis media in children, and to provide reference for clinical treatment. Methods The clinical data of 110 children with OSAHS complicated by otitis media from June 2010 to June 2015 were analyzed retrospectively. Results Compared with the control group, three age groups of the patients in the observation group postoperative tympanogram recovery time were significantly reduced ( P〈0.05 ) ; compared before treatment, investigation of the specific life quality total score and apnea hypopnea index ( AHI ) of patients in the two groups ( OSA-18 ) were decreased significantly, the lowest oxygen saturation ( LSaO2 ) were significantly increased, and the changes of indexes of the three groups of patients more obviously ( P〈0.05 ) ; the observation group with middle ear effusion time was ( 6.1 ±1.4 ) d, significantly less than the control group ( 9.9 ± 2.6 ) d, and the concept of number cases of infection were significantly less than the control group and observation group ( P〈0.05 ) .In the comparison of air conduction threshold, the threshold in the observation group and the control group after treatment were significantly lower than that before treatment, and the air conduction threshold in the observation group was significantly lower than the control group ( P〈0.05 ) . The total effective rate of the group was 94.5%, higher than 76.4% of the control group ( P〈0.05 ) . Conclusion The clinical effect of low temperature plasma knife resection for the treatment of obstructive sleep apnea hypopnea syndrome complicated by otitis media in children has remarkable clinical effect.It can better improve hearing in children in observation group with specific quality of life investigation and respiratory index improvement than in the contrast group.
