血清同型半胱氨酸水平与冠心病相关性分析 被引量:5

Analysis of the correlation between serum homocysteine levels and coronary heart disease

摘要 目的探讨血清同型半胱氨酸(Hcy)水平变化是否为冠心病的独立危险因素,及常见心血管危险项目水平检测对疾病的相关意义。方法急性心梗患者(AMI组)127例,稳定性心绞痛患者(SAP组)91例,对照组55例。测定三组血清Hcy、胱抑素C(Cys-C)、血脂指标水平进行比较分析。对入选对象的传统心血管疾病危险因素采用非条件Logistic回归模型分析。结果 AMI组和SAP组血清Hcy水平明显高于对照组;SAP组血清Cys-C水平显著高于对照组和AMI组;AMI组和SAP组血清CHOL水平显著高于对照组;对照组血清HDL-C水平显著高于两组冠心病组,血清LDL-C水平显著低于两组冠心病组。Hcy水平与HDL-C呈负相关性。高血压史、Hcy为冠心病的独立危险因素,HDL-C为冠心病发生的保护因素。结论 Hcy及Cys-C、CHOL、HDL-C、LDL-C项目的联合检测对冠心病的诊断、病情发展预测及治疗预后有一定的临床意义。 Objective To investigate whether serum homocysteine( Hcy) levels are independent risk factors for coronary heart disease,and explore the significance of the detection of common cardiovascular markers for disease prevention,clinical diagnosis and treatment. Methods 127 acute myocardial infarction( AMI) patients were involved as the AMI group,91 patients with stable angina pectoris( SAP) were collected as SAP group,and healthy control group admitted 55 cases without any disease. The serum Hcy,Cys-C and blood lipids levels among these three groups were detected and compared. Non conditional Logistic regression model was used to analyze the risk factors of traditional cardiovascular disease in the collected objects. Results Serum Hcy levels of the AMI group and the SAP group were obviously higher than that of normal control group. Serum level of Cys-C in SAP group was significantly higher than that of normal control group and the AMI group. Serum CHOL levels of the AMI and the SAP group were significantly higher than that of normal control group. Serum HDL-C level of normal control group was significantly higher than two groups of coronary heart disease,but the serum levels of LDL-C were significantly lower than the two groups of coronary heart disease. Hcy levels had negatively correlation with HDL-C. The history of high blood pressure and Hcy were independent risk factors for coronary heart disease,HDL-C was the protective factor for coronary heart disease. Conclusion The joint detection of Hcy,Cys-C,CHOL,HDL-C,LDL-C has certain clinical significance for the diagnosis of coronary heart disease,disease progression and treatment prognosis.
