摘要
目的:分析儿科革兰阴性菌的耐药性与抗菌药物使用频度(DDDs)的相关性,为临床合理使用抗菌药物提供可参考依据。方法:统计广西贺州市人民医院儿科2012年1月至2016年12月病原菌的分布、耐药性和抗菌药物使用情况,采用Pearson法对革兰阴性菌的耐药性与抗菌药物DDDs的相关性进行分析。结果:20 124份送检标本中共分离出1 426株病原菌,检出率为7.09%;病原菌分布以革兰阴性菌为主,占64.31%,革兰阳性菌和真菌分别占32.68%和3.01%。革兰阴性菌以大肠埃希菌、肺炎克雷伯菌和铜绿假单胞菌为主,对碳青酶烯类、阿米卡星敏感性较高,对其他抗菌药物均有不同程度的耐药,且耐药率呈逐年上升的趋势;主要革兰阴性菌的耐药率与部分常用抗菌药物的DDDs呈明显正相关关系。结论:本院儿科革兰阴性菌耐药形势依然严峻,部分病原菌的耐药水平变化与抗菌药物DDDs存在相关性,应加强临床抗菌药物的使用管理和细菌耐性监测。
Objective: To analyze the correlation between drug resistance of Gram negative bacilli and defined daily doses (DDDs)of antibacterial drugs in Department of Pediatrics, and provide suggestions for rational use of antibacterial agents. Methods: The distribution, drug resistance of Gramnegative bacteria, and the usage of antibiotics in Department of Pediatrics in our hospital from January 2012 to December 2016 were recorded. The correlation between drug resistance to Gramnegative bacteria and the DDDs of antibiotics were analyzed by Pearson method. Results: A total of 1,426 strains of pathogens were isolated in the past five years, with the positive rate of 7. 09%. Gramnegative bacteria were the main pathogenic bacteria and accounted for 64. 31%. Grampositive bacteria and fungus accounted for 32.68% and 3.01%. Gramnegative bacteria mainly consisted of Esch- erichia coli, Klebsiella pneumoniae and Pseudomonas aeruginosa. Gram-negtive bacteria were highly sensitive to carbapenems and amikacin, but resistant to other antibiotics. The resistance rates of mainly Granmegative bacteria were positively correlated to DDDs of commonly used antibiotics. Conclusion: The current situation of drug resistance of Gram negative bacilli in Department of Pediatrics in our hospital still remains severe. There was a correlation between changes in antibiotic resistance of some pathogens and the DDDs of antibiotics. The management of antibiotics use and monitoring of drug-resistant bacteria should be strengthened.
