摘要
目的研究分析2型糖尿病视网膜病变(DR)与动脉粥样硬化(AS)及N-末端脑钠素原(NT-pro BNP)的相关性,以为临床研究和应用提供参考依据。方法选取246例2型糖尿病住院患者,根据患者的视网膜病变情况分组,无视网膜病变者123例为非DR组,有视网膜病变者123例为DR组。分析比较患者的一般资料情况、病史、生活习惯、疾病情况、NT-pro BNP水平、血脂、肾功能相关指标等情况。结果 DR组、非DR组并发AS患者分别为59例(47.97%)、28例(22.76%)。DR组、非DR组NT-pro BNP水平异常偏高者分别62例(50.41%)、20例(16.26%)。DR组与非DR组在体质量指数(BMI)、血脂异常低密度脂蛋白胆固醇(LDL-C)、高脂血症(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、NT-pro BNP水平比较,差异有统计学意义(P<0.05);在性别、年龄、吸烟史、病程、血压比较,差异无统计学意义(P>0.05)。单因素分析结果显示,BMI、并发AS、血脂异常、TC、TG、HDL-C、LDL-C、NT-pro BNP水平均为2型糖尿病患者DR的影响因素。经Logistic回归分析结果发现,T2DM患者视网膜病变与TC、TG、LDL-C和NT-pro BNP水平呈正相关,与HDL-C水平呈负相关。结论 2型糖尿病眼底病变与AS、NT-pro BNP水平有关,其中与TC、TG、LDL-C、NT-pro BNP水平呈正相关,与HDL-C水平呈负相关。
Objective To investigate the relationship among diabetic retinopathy( DR) and atherosclerosis( AS),brain natriuretic peptide( NT-pro BNP) in patients with type 2 diabetes mellitus,and to to provide basis and reference for reasonable application. Methods A total of 246 patients with type 2 diabetes mellitus were enrolled. One hundred and and twenty-three cases without retinopathy were non-DR group and 123 cases with retinopathy were DR group according to the retinopathy of the patients. The general information,history,lifestyle,disease,serum NT-pro BNP levels,blood lipids,renal function related indicators and so on of patients were compared. Results The patients with AS of DR group and non DR group was 59( 47. 97%) and 28( 22. 76%) respectively. The patients with high BNP of DR group and non-DR group was 62 cases( 50. 41%),20 cases( 16. 26%). The difference of BMI,TC,TG,HDL-C,LDLC and NT-pro BNP levels between two groups were statistically significant; the difference of sex,age,smoking history,Blood pressure between two groups were not statistically significant,P > 0. 05. Univariate analysis showed that BMI,concurrent AS,dyslipidemia,TC,TG,HDL-C,LDL-C,NT-pro BNP levels were the risk factors of DR in type 2 diabetes mellitus. The Logistic regression analysis showed that serum TC,TG,LDL-C and NT-pro BNP level were positively correlated with retinal lesions,are negatively correlated with HDL-C level. Conclusion The fundus lesion of type 2 diabetes mellitus is related to the AS and NT-pro B NP,and DR is positively correlated with the TC,TG,LDL-C and NT-pro BNP,is negatively correlated with HDL-C.
