血必净注射液联合参麦注射液对脓毒症急性肺损伤患者疗效及血清HMGB1、sTREM-1的影响 被引量:14

摘要 目的观察血必净注射液联合参麦注射液对脓毒症急性肺损伤患者疗效及血清高迁移率族蛋白B1(HMGB1)、可溶性髓系细胞触发受体-1(s TREM-1)的影响并探讨其机制。方法将78例脓毒症急性肺损伤患者随机分为两组,对照组39例给予血必净注射液治疗,研究组39例给予血必净注射液联合参麦注射液治疗,均连续治疗7 d。比较两组治疗后临床疗效,记录两组治疗前后全身性感染相关器官功能衰竭(SOFA)评分和急性生理学与慢性健康状况Ⅱ(APACHEⅡ)评分,检测两组治疗前后血气分析指标以及血清HMGB1、s TREM-1水平。结果治疗后,研究组临床治疗总有效率显著高于对照组(P<0.05);两组SOFA评分和APACHEⅡ评分均显著降低(P<0.05),且研究组均显著低于对照组(P<0.05);两组动脉血氧饱和度(SaO_2)、动脉氧分压(PaO_2)和氧合指数(PaO_2/FiO_2)水平均较治疗前显著升高(P<0.05),且研究组均显著高于对照组(P<0.05);两组血清HMGB1、s TREM-1水平均较治疗前显著降低(P<0.05),且研究组均显著低于对照组(P<0.05)。结论血必净注射液联合参麦注射液治疗脓毒症急性肺损伤,疗效显著,同时能明显降低血清HMGB1、s TREM-1水平。 Objective: To observe the curative effect of Xuebijing Injection combined with Shenmai Injection and the influence on serum HMGB1 and sTREM-1 in patients with acute lung injury caused by sepsis. Methods: 78 cases of patients with acute lung injury caused by sepsis were randomly divided into two groups: 39 cases in the control group were given Xuebijing Injection treatment;39 cases in the study group were given Xuebijing In- jection combined with Shenmai Injection treatment,and the two groups were treated for 7 days. Clinical efficacy after treatment was compared in the two groups. Systemic infection correlated organ failure (SOFA) score and a- cute physiology and chronic health status ]l (APACHE II ) score before and after treatment were recorded in the two groups. Blood gas analysis index,serum HMGB1 and sTREM-1 levels before and after treatment were detect- ed in the two groups. Results: After treatment ,the total effective rate of the study group was significantly higher than that of the control group (P〈 0.05). SOFA score and APACHE II score were significantly decreased in the two groups (P〈 0.05), and the study group were significantly lower than that of the control group (P〈 0.05). Arte- rial oxygen saturation(SaO2),arterial oxygen partial pressure(PaO2) and oxygenation index(PaOE/FiO2) levels were significantly increased in the two groups (P〈 0.05),and the study group were significantly higher than that of the control group(P〈 0.05). Serum HMGB1 and sTREM-1 levels were significantly decreased in the two groups (P〈 0.05), and the study group were significantly lower than that of the control group (P〈 0.05). Conclusion: Xuebi- ring Injection combined with Shenmai Injection on the patients with acute lung injury caused by sepsis has a sig-nificant effect,and can obviously reduce serum HMGB1 and sTREM-1 levels.
