摘要
目的:探讨首发强迫症患者海马体积及侧脑室下静脉的变化,并分析两者间可能存在的联系。方法:利用3D快速小角度激发序列(3D-FLASH)形态扫描、磁敏感加权成像(SWI)技术对20例健康志愿者及60例首发强迫症患者(按病情分为轻、中、重3组)进行海马检测,计算海马体积并测量SWI图像上侧脑室下静脉的最大长度、直径及分支数目,对比分析海马体积及其静脉系统的变化规律。结果:轻度强迫症组双侧海马体积较对照组差异无统计学意义(P>0.05),中度和重度强迫症组双侧海马体积均较对照组明显缩小(P<0.01)。轻度强迫症组侧脑室下静脉长度、最大直径较对照组明显减小(P<0.01),分支数量间差异无统计学意义(P>0.05);与对照组比较,中、重度强迫症组侧脑室下静脉长度、最大直径及分支数量显著减小,差异具有统计学意义(P<0.01和P<0.05)。结论:联合运用形态学测量及SWI技术可发现不同病情强迫症患者海马结构的变化规律。
Objective: To investigate the changes of hippocampal volume and inferior vena cava in patients with first-episode obsessive-compulsive disorder,and to analyze the potential relationship between them. Methods: Sixty first-episode obsessive-compulsive disorder patients( divided into mild,moderate and major group as the condition of the severity of obsessive-compulsive disorder) and 20 healthy controls were scanned with the 3-Dimensional fast low angle shot imaging sequence( 3 D-FLASH) and SWI. The hippocampal volumes were calculated,the maximum diameter,length and the branching numbers of the inferior ventricular vein clearly demonstrated in the SWI images were measured. Then the regularity of the changes in the venous system and morphology among the three different severity of obsessive-compulsive disorder and healthy controls were contrastively analyzed. Results: Compared with the control group,the volume of bilateral hippocampus in mild obsessive-compulsive disorder group was not statistically significant( P > 0. 05); the volume of bilateral hippocampus in moderate and severe obsessive-compulsive disorder group was significantly lower than that in control group( P < 0. 01). Compared with the control group,the number of branches and the diameter of the the lateral ventricle vein in the mild obsessive-compulsive disorder group was decreased( P < 0. 01),but there was no statistical difference between the number of venous branches( P > 0. 05); compared with the control group,the length of the lateral ventricle vein,the maximum diameter and the number of branches in moderate and severe obsessive-compulsive disordergroup decreased significantly( P < 0. 01 or P < 0. 05). Conclusion: The combination of the SWI and morphological measurement may be used to detect the regularity of the changes in hippocampal structure in patients with different severity of obsessive-compulsive disorder.
