摘要
目的探讨不同干预方式对妊娠期糖尿病(GDM)妊娠结局的影响。方法将我院90例健康孕妇(对照组)与165例妊娠期糖尿病孕妇,随机分成试验A组(82例)和试验B组(83例)经不同干预后,观察对照组、试验A组与B组的分娩方式、围生儿结局和并发症发生情况。结果对照组剖宫产率显著低于试验A组和试验B组(P<0.05),另试验A组的剖宫产率要高于试验B组(P<0.05)。对照组与试验A、B两组比较,其围生儿不良结局和产妇并发症的发生比率均小于试验组(P<0.05),试验A组围生儿巨大儿、低血糖发生率、产妇妊高症、羊水过多发生率均明显高于试验B组(P<0.05)。结论妊娠期糖尿病对产妇的妊娠结局、围生儿结局和产妇并发症的发生都有着不利影响,妊娠期间应定期检测血糖,及早诊断、发现妊娠期糖尿病,并对其采取切实可行的干预措施,确保母婴安全。
Objective To invesligale lhe influence of differenl inlervenlion melhods on geslalional diabeles mellilus （ GDM） pregnancy oulcome. Methods Ninely cases of heallhy pregnanl women were selecled as lhe conlrol group, 165 cases of diabelic pregnanl women were selecled and randomly divided inlo experimenlal group A （ n=82） and experimenlal group B （ n =83） according lo differenl inlervenlion melhods. The mode of delivery, perinalal oulcomes and complicalions of lhree groups were compared. Results The rale of cesarean seclion in lhe conlrol group was significanlly lower lhan lhal in lhe experimenlal group A and lhe experimenlal group B （ P 〈0.05）, and lhe rale of cesarean seclion in experimenlal group Awas significanlly higher lhan lhal in experimenlal group B （ P〈0.05）. The incidence rale of poor perinalal oulcomes and malernal complicalions of lhe conlrol group were beller lhan lhose of lhe experimenlal group A and experimenlal group B （ P 〈0.05）. The incidence rale of poor perinalal oulcomes on felal macrosomia, hypoglycemia and malernal complicalions on pregnancy- induced hyperlension syndrome and polyhydramnios of lhe experimenlal group A were higher lhan lhose of lhe experimenlal group group B （ P〈0.05）. Conclusion Geslalional diabeles has adverse effecls on pregnancy oulcomes, perinalal oulcomes and lhe incidence of malernal complicalions. Ilis necessary lo lake regular blood glucose during pregnancy, early diagnosis and deleclion of geslalional diabeles mellilus. Taking feasible inlervenlion measures can ensure lhe safely of lhe molher and child.
