血清25 (OH) D3水平与哮喘患儿炎症反应及肺功能的关系 被引量:4

Relationships between serum 25 (OH) D_3 levels and inflammatory response and pulmonary function of children with asthma

摘要 目的探讨血清25-羟维生素D3[25(OH)D3]水平与哮喘患儿炎症反应及肺功能的关系。方法选择2015年3月-2016年3月该院接受治疗的哮喘患儿500例作为观察组,另取同期体检的健康儿童100例作为对照组。分别对比两组儿童的血清细胞因子、肺功能、血清25(OH)D3、血钙、血磷和碱性磷酸酶水平,并应用Pearson相关性分析分析相关性。结果观察组儿童血清25(OH)D3水平显著低于对照组,差异有统计学意义(t=2.786,P<0.05),两组儿童血钙、血磷和碱性磷酸酶水平比较差异无统计学意义(t=0.483、0.436、0.728,均P>0.05)。观察组儿童血清肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)、γ-干扰素(INF-γ)水平均显著高于对照组,差异有统计学意义(t=2.887、3.017、2.803,均P<0.05)。观察组儿童第一秒用力呼气容积(FEV1)、呼气流量峰值(PEF)、一秒用力呼气量占用力肺活量比值(FEV1/FVC)水平均显著低于对照组,差异有统计学意义(t=2.726、2.775、3.009,均P<0.05)。经Pearson相关性分析可得,血清25(OH)D3水平与TNF-α、IL-6、INF-γ呈负相关(r值分别为-0.694、-0.752、-0.542,均P<0.05),与FEV1、PEF及FEV1/FVC呈正相关(r值分别为0.573、0.719、0.543,均P<0.05)。结论哮喘患儿血清25(OH)D3水平下降,与患儿炎症反应及肺功能密切相关,临床治疗中可通过提高患儿血清25(OH)D3水平,以改善炎症反应程度及肺功能。 Objective To explore the relationships between serum levels of 25( OH) D3 and inflammatory response and pulmonary function of children with asthma. Methods A total of 500 children with asthma treated in the hospital from March 2015 to March 2016 were selected as observation group,and 100 healthy children receiving healthy physical examination in the hospital during the same period were selected as control group. The levels of serum cytokines,pulmonary function,serum 25( OH) D3,calcium,phosphorus,and alkaline phosphatase in the two groups were compared. Pearson correlation analysis was used to analyze the correlation. Results The level of serum25( OH) D3 in observation group was statistically significantly lower than that in control group( t = 2. 786,P0. 05). There was no statistically significant difference in the levels of serum calcium,phosphorus,and alkaline phosphatase between the two groups( t = 0. 483,0. 436,0. 728,all P0. 05). The levels of serum tumor necrosis factor-α( TNF-α),interleukin-6( IL-6),and γ-interferon( INF-γ)in observation group were statistically significantly higher than those in control group( t = 2. 887,3. 017,2. 803,all P0. 05). The levels of the forced expiratory volume in the first second( FEV1),peak expiratory flow( PEF),FEV1/FVC in observation group were statistically significantly lower than those in control group( t = 2. 726,2. 775,3. 009,all P 0. 05). Pearson correlation analysis showed that serum25( OH) D3 level was negatively correlated with TNF-α,IL-6,and INF-γ levels,respectively( r =-0. 694,-0. 752,-0. 542,all P0. 05),while it was positively correlated with FEV1,PEF,and FEV1/FVC,respectively( r = 0. 573,0. 719,0. 543,all P 0. 05).Conclusion The levels of serum 25( OH) D3 decrease in children with asthma,which is closely correlated with inflammatory response and pulmonary function of these children,the degree of inflammatory response and pulmonary function can be improved by raising serum25( OH) D3 level
