52例先心病患儿EEG和BAEP检测的临床分析 被引量:2

摘要 目的 通过小儿先心病 (CHD)脑电图 (EEG)、脑干听觉诱发电位 (BAEP)的变化 ,评价CHD对患儿脑功能、脑干发育的影响。方法 分析了 5 2例儿童CHDEEG、BAEP的变化。结果 CHD小儿EEG异常率为 4 2 .3% ,其中非紫绀组为 2 6 .3% ,紫绀组为 85 .7% ,两组间差异显著 (P <0 .0 1)。BAEP异常率 6 3.5 % ,非紫绀组为 5 2 .6 % ,紫绀组为 92 .9% ,两组间差异非常显著 (P <0 .0 1)。Ⅴ波潜伏期 (PL)、Ⅰ~Ⅴ波峰间潜伏期 (IPL)紫绀组较健康儿显著延长 (P <0 .0 5 ,P <0 .0 1,)。结论CHD可损害脑功能并延迟脑干发育 ,早期干预可能是必要的。 Objective Electroencephalographic(EEG) and brainstem auditory evoked potential(BAEP) were analysised in the interest of evaluating the effect of congenital heart disease(CHD) on cerebral function and brainstem maturation.Methods EEG and BAEP were analysised in 52 children who had CHD. Results Abnormal incidence of EEG occurred in 22 (42.3%) of 52 CHD children, occurred in 12(85.7% ) of 14 in cyanotic group and in 10(26.3%) of 38 in non cyanosis group and significant differences were seen between groups( P <0 .01). Abnormal incidence of BAEP occurred in 33(63.5%) of 52 CHD children, occurred in 13(92.9%)of 14 in cyanotic group and in 20 of 38 (52.6%)in non cyanosis group and significant differences were seen between groups( P <0 .01). It was more prolonged in V peak latencies (PL) of cyanosis than controls ( P <0 .05),and in I-V interpeak latencies( IPL) of cyanosis than controls( P < 0.01) . Conclusion The data indicate that CHD may damage cerebral function and retarded brainstem maturation and the interventions were essential in early stage. The BAEP may be more sensitive than EEG in providing information of the central nervous system damaged by CHD.
