脊柱结核患者入院手术与出院后两种健康教育模式的效果分析 被引量:8

摘要 目的 探讨不同时间点护理健康教育模式对脊柱结核患者心理状态及服药依从性的影响.方法 选取2016年1-12月入住解放军第三○九医院且符合纳入标准的脊柱结核患者192例,按入住2个不同的病区分为观察组(96例,接受不同时间点护理健康教育模式)和对照组(96例,接受传统护理健康教育模式).使用焦虑自评量表(SAS)、抑郁自评量表(SDS)、服药依从性量表(MMAS-8)于出院时、出院后第1和3个月末复查时对两组患者进行心理状态和服药依从性评估.结果 观察组在出院时、出院后第1和3个月末时焦虑评分[(42.8±10.7)分、(41.9±12.5)分、(40.6±10.5)分]明显低于对照组[(49.5±13.7)分、(48.5±13.6)分、(48.9±12.3)分],差异均有统计学意义(t=2.25,P=0.031;t=-2.23,P=0.024;t=-2.42,P=0.028),抑郁自评量表评分[(46.1±12.8)分、(44.9±10.9)分、(44.5±12.6)分]明显低于对照组[(53.3±13.5)分、(50.7±11.6)分、(51.2±13.7)分],差异均有统计学意义(t=-2.96,P=0.021;t=-2.36,P=0.029;t=-2.45,P=0.032);观察组在出院后第1和3个月末的服药依从率[97.92%(94/96)和96.88%(93/96)]明显高于对照组[88.54%(85/96)和89.58%(86/96)],差异均有统计学意义(x2=6.25,P=0.026;x2 =5.21,P=0.023).结论 不同时间点护理健康教育模式可以缓解脊柱结核患者焦虑及抑郁状态,改善心理状态,提高服药依从性. Objective To explore the influence of health education program on psychological status and medication compliance of patients with spinal tuberculosis at different stages.Methods From January to December,2016 in 309th Hospital of Chinese People's Liberation Army,192 cases from two different wards were divided into observation group (96 cases,receiving nursing health education programs at different time points) and control group (96 cases,accept the traditional model of nursing health education).Results Scores at the end of the first,third month at the time of discharge and after discharge anxiety in observation group ((42.8± 10.7),(41.9± 12.5),(40.6±10.5)) were significantly lower than those in control group ((49.5±13.7),(48.5±13.6),(48.9±12.3))(t=2.25,P=0.031;t=-2.23,P=0.024;t=2.42,P=0.028);and so were the self rating depression scale score ((46.1±12.8)vs.(53.3±13.5),t=-2.96,P=0.021;(44.9±10.9)vs.(50.7±11.6),t=-2.36,P=0.029;(44.5±12.6) vs.(51.2±13.7),t=-2.45,P=0.032).The compliance rates 1-,3-month after treatment in observation group were significantly higher than those in control group (97.92% (94/96) vs.88.54% (85/96),x2=6.25,P=0.026;96.88% (93/96) vs.89.58% (86/96),x2=5.21,P=0.023).Conclusion At different times,the nursing health education program can alleviate the anxiety and depression status of the patients with spinal tuberculosis,and improve the psychological status and improve the compliance of medication.
