摘要
目的探讨整体护理干预对颅内动脉瘤(ICA)患者行血管内治疗(EVT)术的影响。方法选择2009年1月—2013年12月在太和医院行EVT的ICA患者298例。应用随机数字表法,将患者分为整体护理组172例与传统护理组126例。对传统护理组实施传统护理干预,对整体护理组实施整体护理干预。分析、比较两组状态焦虑水平、患者满意度、健康教育覆盖率、医护配合度以及术后并发症发生率。结果 1整体护理组状态焦虑水平手术当天评分为(53.26±5.47),出院当天评分为(39.66±4.06),与传统护理组评分(71.52±7.09)、(57.65±5.63)分别比较,明显下降(P<0.01);2整体护理组出院时患者满意度、健康教育覆盖率和医护配合度分别为94.77%、99.42%、97.67%,与传统护理组87.30%、89.68%、86.51%比较,有明显提高(P<0.05);3整体护理组患者穿刺点局部血肿、脑血管痉挛等并发症发生率明显低于传统护理组(P<0.05)。结论整体护理可以降低ICA患者的负性心理和手术并发症的发生,提高护理质量。
[ Objective] To explore the effects of holistic nursing intervention on intracranial aneurysm （ICA） patients undergoing endovascular treatment （EVT）. [ Methods] A total of 298 ICA patients hospitalized in Taihe hospital from January 2009 to Decem- ber 2013 and treated by EVT were enrolled. The patients were divided into holistic nursing group of 172 cases and traditional nurs- ing group of 126 cases by random number table. The holistic nursing intervention was performed in holistic nursing group. The traditional nursing intervention was performed in traditional nursing group. The level of state anxiety, patient satisfaction, health education coverage, the degree of coordinate with doctors and nurses and incidence of postoperative complications were analyzed and compared. [Results] （1）The level of state anxiety on the day of surgery was （53.26 ± 5.47） and the day of discharge was （39.66 ± 4.06） in holistic nursing group, compared with the traditional nursing group （ 71.52 ± 7.09 and 57.65 ± 5.63） , the scores decreased significantly （P 〈 0.01 ）. （2） Patient satisfaction, health education coverage, the degree of coordinate with doctors and nurses in holistic nursing group were 94.77%, 99.42% and 97.67% respectively, compared with the traditional nursing group （87.30%, 89.68% and 86.51% ） , the rates increased significantly （P 〈 0.05）. （3）The incidence of surgical complications of lo- cal hematoma in puncture point and cerebral vasospasm in holistic nursing group were significantly lower than those in traditional nursing group （ P 〈 0.05 ）. [ Conclusion] Holistic nursing can reduce the incidence of negative psychological and surgical complications in ICA patients, improve the quality of nursing.
