摘要
目的探讨脑苷肌肽联合奥拉西坦治疗卒中后血管性痴呆(VD)的疗效及对其认知功能、脑神经递质水平的影响。方法选择本院自2014年1月至2016年6月收治的卒中后VD患者78例,随机分为观察组与对照组各39例。两组均予以常规治疗,对照组加用奥拉西坦,观察组在对照组基础上加用脑苷肌肽。对比两组认知功能量表(MMSE、Mo CA)评分、Barthel指数(BI)、神经功能缺损(NIHSS)评分及血清神经元特异性烯醇化酶(NSE)、脑源性神经营养因子(BDNF)水平变化。结果治疗后观察组的MMSE、Mo CA及BI评分均显著高于对照组,NIHSS评分显著低于对照组(P<0.05);治疗后观察组的P300潜伏期明显短于对照组,波幅明显高于对照组(P<0.05);治疗后观察组的血清NSE显著低于对照组,而BDNF水平显著高于对照组(P<0.05)。结论脑苷肌肽联合奥拉西坦能够有效促进卒中后VD患者认知功能及神经功能的康复,其机制可能与其调节血清NSE、BDNF水平有关。
Objective To investigate the effect of cattle encephalon glycoside and lgnotin injection combined with oxiracetam in the treatment of patients with vascular dementia ( VD ) after stroke and its influence on the cognitive function and serum level of NSE, and BDNF. Method Seventy-eight patients with VD after stroke during form January 2014 to June 2016 were randomly divided into observation group and control group ( thirty-night cases for each group ) . All the patients received routine treatmemt, desides the control group were given oxiracetam, while the observation group were given cattle encephalon glycoside and lgnotin injection on the basis of control group. The changes of scores of cognitive function scale ( MMSE, MoCA ) , Barthel index ( BI ) , neurological deficit ( NIHSS ) and serum levels of NSE and BDNF were compared between the two groups. Results After treatment, the MMSE, MoCA and BI scores in the observation group were significantly higher than those of the control group, the NIHSS score was significantly lower ( P〈0.05 ) . After treatment,the P300 latency in the observation group was significantly shorter than that in the control group,while the amplitude was significantly higher ( P〈0.05 ) ; After treatment, the serum level of NSE in the observation group was significantly lower than that of the control group, while the level of BDNF was significantly higher ( P〈0.05 ) .Conclusion Cattle encephalon glycoside and lgnotin injection combined with oxiracetam can promote the recovery of cognitive function and neurological function in YD patients after stroke,its mechanism may be related to the regnlation of serum levels of NSE and BDNF.
