早期康复训练对急性脑梗死患者运动功能及血清脑源性神经营养因子水平的影响 被引量:28

Effect of early rehabilitation training on motor function and the level of serum brain-derived neurotrophic factor in patients with acute cerebral infarction

摘要 目的探讨早期康复训练对急性脑梗死(ACI)患者肢体运动功能及血清脑源性神经营养因子(BDNF)水平的影响。方法选择2015年1月至2016年1月黄冈市中心医院收治的ACI患者90例,将患者分为观察组和对照组,每组45例。对照组患者仅给予常规药物治疗,观察组患者同时给予早期康复训练,分别于治疗前后采用酶联免疫吸附双抗体夹心法检测血清BDNF水平,采用美国国立卫生研究院卒中量表(NIHSS)评估患者神经功能缺损情况,采用Fugl-Meyer运动功能评分法(FMA)评估患者肢体运动功能,采用Barthel指数评分评定患者日常生活能力(ADL),对2组患者血清BDNF水平、NIHSS评分、FMA评分及ADL评分进行比较。结果观察组和对照组患者治疗有效率分别为86.67%(39/45)、66.67%(30/45),观察组患者治疗有效率显著高于对照组(χ~2=8.332,P<0.05)。治疗前2组患者神经功能缺损NIHSS评分比较差异无统计学意义(P>0.05);2组患者治疗后神经功能缺损NIHSS评分显著低于治疗前(P<0.05);治疗后观察组患者神经功能缺损NIHSS评分显著低于对照组(P<0.05)。治疗前2组患者血清BDNF水平比较差异无统计学意义(t=1.030,P>0.05);2组患者治疗后血清BDNF水平显著高于治疗前(P<0.05);治疗后观察组患者血清BDNF水平显著高于对照组(P<0.05)。治疗前2组患者FMA评分、ADL评分比较差异均无统计学意义(P>0.05);2组患者治疗后FMA评分、ADL评分显著高于治疗前(P<0.05);治疗后观察组患者FMA评分、ADL评分显著高于对照组(P<0.05)。结论早期康复训练可有效促进ACI患者血清BDNF表达,改善患者神经功能缺损、肢体运动功能及ADL,提高治疗效果。 Objective To explore the effect of early rehabilitation training on motor function and the level of serum brain-derived neurotrophic factor(BDNF) in patients with acute cerebral infarction (ACI). Methods A total of 90 patients with ACI were selected in Huanggang Central Hospital from January 2015 to January 2016. The patients were divided into ob- servation group and control group ,45 cases in each group. The patients in the control group were treated with conventional med- ication, and the patients in the observation group were treated with early rehabilitation training on the basis of conventional medication. The level of serum BDNF was detected by enzyme-linked immunosorbent double antibody sandwich method, the neurologic impairment was assessed by the national institutes of health stroke scale (NIHSS) ,the limb motor function was as- sessed by Fugl-Meyer assessment(FMA) score, and the activity of daily living (ADL) was assessed by Barthel index score be- fore and after treatment. The serum BDNF level, NIHSS score, FMA score and ADL score were compared between the two groups. Results The effective rate of in the observation group and control group was 86.67% ( 39/45 ) and 66.67% ( 30/45 ) respectively, the effective rate in the treatment group was significantly higher than that in the control group (χ2 = 8. 332, P 〈 0.05 ). There was no significant difference in NIHSS score between the two groups before treatment ( P 〉 0.05 ). The NIHSS score of neurologic impairment after treatment was significantly lower than that before treatment in the two groups (P 〈 0.05 ). The NIHSS score of neurologic impairment in the observation group was significantly lower than that in the control group after treatment ( P 〈 0.05 ). There was no significant difference in serum BDNF level between the two groups before treatment ( t = 1. 030,P 〉0.05). The serum BDNF level after treatment was significantly higher than that before treatment in the two groups ( P 〈 0. 05 )
