摘要
目的评价己烯雌酚对绝经后妇女糖尿病性视网膜病变保护作用。方法筛选2015年7月—2017年1月,眼科收治的绝经后妇女糖尿病性视网膜病变患者96例,按照随机数字表达法分为观察组和对照组,每组各48例,对照组采用常规糖尿病治疗管理,补充维生素。观察组,在对照组基础上,联合己烯雌酚治疗,持续3周。治疗前后进行眼底荧光血管造影(FFA)、光学相干断层成效(OCT)视力等检查。对比治疗前后最佳矫正视力(best corrected visual acuity,BCVA)、视网膜厚度、眼内压水平。观察两组治疗中不良反应。结果治疗3周后,观察组与对照组视网膜厚度、眼压低于治疗前、观察组低于对照组,两组对象BCVA高于治疗前,观察组高于对照组,差异具有统计学意义(P<0.05)。观察组并发黄斑变性、视网膜中央动静脉阻塞、视网膜中心血管出血发生率与对照组差异无统计学意义(P>0.05)。观察组不良反应发生率与对照组异常无统计学意义(P>0.05)。结论己烯雌酚可防治绝经后妇女糖尿病性视网膜病变,但需预防不良反应发生风险,提高合理用药水平。
Objective To evaluate the protective effect of diethylstilbestrol on diabetic retinopathy in postmenopausal women. Methods Screening in July 2015-January 2017,96 cases of patients with diabetic retinopathy in postmenopausal women in the hospital from the post,according to the random number expression were divided into observation group and control group,48 cases in each group,the control group uses the conventional treatment of diabetes management,vitamin supplements. The observation group was treated with diethylstilbestrol for 3 weeks on the basis of the control group. Fundus fluorescein angiography（ FFA）,optical coherence tomography（ OCT）,visual acuity,et al were performed before and after treatment. The best corrected visual acuity（ best,corrected,visual,acuity,BCVA）,retinal thickness and intraocular pressure level were compared before and after treatment. Adverse reactions were observed in the two groups. Results After 3 weeks of treatment,the observation group and the control group,intraocular pressure,retinal thickness lower than that before treatment,the observation group than the control group,two groups of BCVA was higher than that before treatment,the observation group than the control group,the difference was statistically significant（ P〈 0. 05）. The incidence of macular degeneration,central retinal vein occlusion,and central retinal vascular bleeding in the observation group was not significantly different from that in the control group（ P〉 0. 05）. The incidence of adverse reactions in the observation group was not statistically significant compared with the control group（ P〉 0. 05）. Conclusion Diethylstilbestrol can prevent and treat diabetic retinopathy in postmenopausal women,but it is necessary to prevent the risk of adverse reactions and improve the level of rational use of drugs.
