摘要
目的分析妊娠合并卵巢肿瘤及瘤样病变的诊断方法、病理类型、手术时机和妊娠结局,以指导临床诊疗。方法该院2010年1月—2016年5月期间共收治妊娠合并卵巢肿瘤及瘤样病变136例,分析相关资料,探讨诊断方法、手术方式(腹腔镜手术或开腹手术)及时机、并发症、妊娠结局等指标。结果卵巢成熟性畸胎瘤最为多见,为76例(55.88%),其次是卵巢囊腺瘤43例(31.62%),恶性肿瘤(上皮性肿瘤)3例(2.21%),其他病理类型占(10.29%)。常见并发症是卵巢肿瘤蒂扭转,需急诊手术探查,50%卵巢坏死行患侧附件切除术。中期妊娠合并卵巢良性肿瘤可考虑腹腔镜探查,晚期妊娠及恶性肿瘤需开腹手术。结论妊娠合并卵巢肿瘤及瘤样病变大部分为良性病变,无需过多干预。对于合并卵巢恶性肿瘤,应及时终止妊娠,进行恶性肿瘤的综合治疗。
Objective To analyze the diagnostic method, pathological type, operation opportunity and pregnancy outcome of pregnancy and ovarian tumor and tumor-like lesion to guide the clinical diagnosis and treatment. Methods 136 cases of patients with pregnancy and ovarian tumor and tumor-like lesion in our hospital from January 2010 to May 2016 were selected and the related data were analyzed and the diagnostic method, operation method（laparoscopic surgery or laparotomy）,opportunity, complication and pregnancy outcome were studied. Results The benign ovarian teratomas were the most common with 76 case（55.88%）, and then 43 cases with ovarian cystadenoma（31.62%）, 3 cases with malignant tumors（epithelial ovarian tumor）（2.21%） and the other pathological type was 10.29%, and the common complication was ovarian tumor torsion that needs the emergency operation exploration, and 50 percent of patients with ovarian necrosis underwent the affected side appendectomy, and the laparoscopic approach could be considered in treatment of mid-trimester pregnancy and benign ovarian tumor, and the advanced pregnancy and malignant tumors needed the laparotomy. Conclusion Most of the patients with pregnancy and ovarian tumor and tumor-like lesion are the benign lesions, without excessive intervention. We should terminate a pregnancy in time and conduct the comprehensive treatment of malignant tumors for patients with malignant ovarian neoplasm.
