摘要
目的:探讨腹腔镜手术联合戈舍瑞林(促性腺激素释放激素激动剂,GnRH-a)治疗卵巢子宫内膜异位囊肿患者临床疗效。方法:选择2014年10月-2015年10月于本院接受治疗的卵巢子宫内膜异位囊肿患者90例作为研究对象,将其随机分为对照组和GnRH-a组,每组各45例。对照组患者采用腹腔镜手术治疗,GnRH-a组术后加用GnRH-a 3~6个月。随访观察1年,在相同的时间点比较两组患者的治疗效果。结果:GnRH-a组患者复发率为22.2%显著低于对照组31.1%(P<0.05);手术后半年,GnRH-a组患者的黄体生成素(LH)、血清卵泡刺激素(FSH)、雌激素(E_2)水平明显低于对照组(P<0.05),而两组患者基础状态卵泡数、卵巢体积比较,差异均无统计学意义(P>0.05);术后一年,GnRH-a组妊娠率、再次妊娠间隔时间均明显优于对照组(P<0.05),两组患者月经情况比较差异均无统计学意义(P>0.05)。结论:腹腔镜手术联合戈舍瑞林治疗子宫内膜异位囊肿在控制复发率、激素水平、基础状态卵泡数、卵巢体积变化、对再妊娠的影响以及影响月经等方面的临床疗效均优于单纯采用腹腔镜手术治疗。
Objective：To investigate the clinical efficacy of laparoscopic surgery combined with GnRH-a in the treatment of endometrial cysts in the ovary.Method：90 cases of endometrial cysts were treated in our hospital from October 2014 to October 2015 were selected as the subjects,they were randomly divided into control group and GnRH-a Group,45 cases in each group.The control group was treated with laparoscopic surgery.GnRH-a group was treated with GnRH-a after operation.One year follow-up observation,the therapeutic effect of the two groups was compared at the same time.Result：The recurrence rate of patients in GnRH-a group was 22.1%,significantly lower than that control group of 31.1% （P〈0.05）.After operation 6 months,the levels of serum luteinizing hormone（LH）,serum follicle stimulating hormone（FSH） and estrogen（E2） in GnRH-a Group groups were compared,the differences were not statistically significant（P〉0.05）.1 year after operation,the rate of pregnancy and the interval of repregnancy in GnRH-a group were obviously better than those of control group（P〈0.05）,the menstrual status of two groups were compared,the differences were not statistically significant（P〉0.05）.Conclusion：The clinical curative effect of laparoscopic surgery combined with goserelin treatment of endometrial cysts in the control of recurrence rate,hormone level,basal follicle number,ovarian volume changes,effects on pregnancy and menstruation are better than the simple use of laparoscopic surgery.
