摘要 目的探讨超声联合MRI技术在胎儿孤立性侧脑室扩张诊断中的临床应用价值。方法回顾分析2014年1月至2016年12月南方医科大学南方医院妇产科超声诊断"孤立性侧脑室扩张"或可疑合并中枢神经系统疾病的单胎妊娠孕妇86例,比较超声和MRI检查结果的一致性。结果 (1)MRI与超声对胎儿侧脑室测量分组间具有中度一致性(κ=0.603);(2)MRI检查确诊超声疑似诊断6例;增加超声诊断信息9例:1例部分性胼胝体发育不全,1例胼胝体发育不良合并中线脂肪瘤,3例颅内囊肿(左侧侧脑室额角旁室管膜下类囊肿病变,双侧侧脑室前角囊肿,室管膜下囊肿),4例脑发育不良(双侧顶叶、枕叶发育不良;脑发育不良合并室管膜下囊肿;双侧小脑半球发育不对称;脑白质发育不良)。结论通过超声联合MRI技术对孤立性侧脑室扩张胎儿疾病的准确识别,可在判断妊娠预后和临床管理中发挥重要作用。 Objective To evaluate the clinical value of prenatal MRI in fetal with isolated mild ventriculomegaly( IVM) diagnosed by ultrasonography. Methods 86 cases of single pregnant women were recruited who were diagnosed as IVM by ultrasonography or suspected central nervous system diseases from January 2014 to December 2016 in Nan Fang Hospital, Southern Medical University.Findings of MRI were compared with those of ultrasonography. Results( 1) MRI had moderate consistence with ultrasonography in measurement of fetal lateral ventricle( κ = 0. 603);( 2) MRI confirmed 5 suspected diagnosis cases of ultrasound examination; completed 8 cases in ultrasonic diagnostic information: 1 case of partial agenesiacorporiscallos, 1 case of agenesis of corpus callosum associated with midline lipoma; 3 cases of intracranial cysts( cystic lesions on the left lateral ventricle subependymal,bilateral anterior horn of lateral ventricle cyst, subependymal cyst),4 cases of cerebral dysplasia( bilateral parietal lobe and occipital lobe dysplasia; brain dysplasia associated with subependymal cysts; bilateral cerebellar hemisphere dysplasia; white matter dysplasia). Conclusions Ultrasonography combined with MRI can be used to accurately identify the prognosis of pregnancy and clinical management of isolated mild ventriculomegaly.
