256层CT高心率冠状动脉成像扫描时相选择研究 被引量:5

摘要 目的:探讨256层CT高心率冠状动脉成像的时相选择的方法,前瞻性心电门控或回顾性心电门控。方法:收集2014.1-2014.6在我科行256层冠状动脉CTA回顾性心电门控检查(RECGT-CTA)的病人264例(心率稳定组253例,心率不稳定组11例),重建收缩期、舒张期冠状动脉图像,研究优良率与心率(65-75/76-85/86次/分以上)、时相(单纯收缩期/单纯舒张期/既包括收缩期又包括舒张期)、性别、年龄(31-50/51-60/61-84岁)的关系。结果:1、心率稳定组冠状动脉图像总体优良率(收缩期加舒张期)92.49%(234/253),不稳定组总体优良率54.55%(6/11),(P=0.001)。2、稳定组总体优良率92.49%,收缩期(单纯收缩期+收缩期并舒张期)优良率83.4%(211/253),舒张期(单纯舒张期+收缩期并舒张期)优良率67.98%(172/253)(P=0.001)。3、随心率(65-75/76-85/86次/分以上)上升,稳定组总体优良率下降,96.06%(122/127)/91.67%(77/84)/83.33%(35/42),(P=0.035)。全时相(收缩期与舒张期同时)优良率下降,77.95%(99/127)/47.62%(40/84)/23.81%(10/42),(P=0.001)。收缩期优良率上升,6.30%(8/127)/34.52%(29/84)/59.52%(25/42),(P=0.001)。心率在65-75次/分时,男性舒张期优良率高于女性(P=0.043)。4、各年龄段(31-50/51-60/61-84岁)图像质量优良率差异无统计学意义(P=0.51)。结论:心率65-85次/分时,可用256层CT PECGT-CTA收缩期采集,其中心率65-75次/分且为男性者宜用PECGT-CTA舒张期采集;心率大于85次/分时,推荐RECGT-CTA收缩期采集。心率不稳患者,建议进行药物控制后,待心率稳定后(心率变化小于10次/分)再进行扫描。 To investigate the phase selecting method for 256 slice CT of cardiac coronary angiography (CCA),PECGT-CTA (prospective ECG-triggered CT angiography) or RECGT-CTA (retrospective ECGtriggered CT angiography).Methods: The systolic and diastolic phase CCA images of 264 cases (253 patients with stable heart rate and 11 patients with unstable heart rate) between 2014.1-2014.6 in our hospital were reconstructed,and evaluated their image excellent and good rate correlation with heart rate (65-75/76-85/86 beats/min),phase [systolic/diastolic/both systolic and diastolic (whole phase) ],age (31-60/61-84 years).Results: the rate of excellent and good rate in heart rate stable group was 92.49% (234/253),the rate of excellent and good rate in unstable group was 54.55% (6/11),(X2= 14.061,P = 0.001).In stable group,the excellent and good rate in all systolic phase (simple systolic plus systolic and diastolic) was 83.4% (211/253),in diastolic phase (simple diastolic plus both systolic and diastolic) was67.98% (172/253),(X2= 16.34,P = 0.001).All excellent and good rates in heart rate stable group decreased as the heart rate increased,96.06% (122/127)/91.67% (77/84)/83.33% (35/42),(X2= 6.733,P = 0.035).The excellent and good rates of the whole phase were decreased,77.95% (99/127)/47.62% (40/84)/23.81% (10/42),(X2=44.821,P = 0.001).The excellent and good rates of systolic phase were increased,6.30% (8/127)/34.52% (29/84)/59.52% (25/42),(X2= 55.15,P = 0.001).The heart rate in 65-75/min,the excellent and good rate of diastolic phase in male was higher than that in female,(x^2= 4.089,P = 0.043).There were no significant differences in the excellent and good rates for different age groups (31-50/51-60/61-84 years),(X^2= 5.27,P = 0.51).Conclusion: The systolic acquisition of PECGT-CTA is available for CCA of 256 slice CT in patient with heart rate 65-85 beats/min,while the diastolic acquisition of PECGT-CTA will
