摘要
目的探讨肠内营养疗法对炎症性肠病(IBD)患者紧密连接蛋白(occludin)、二胺氧化酶(DAO)、脂多糖(LPS)与分泌型免疫球蛋白A(s Ig A)水平的影响。方法选取2013年1月至2016年7月期间IBD患者116例,随机分为两组,各58例。对照组采用免疫肠内营养,观察组采用生态免疫肠内营养。采用免疫比浊法测定血清清蛋白(ALB)与前清蛋白(PA)水平,采用免疫组化法测定肠黏膜occludin蛋白与肠黏膜浆细胞s Ig A表达含量,采用改良的偶氯基质显色法鲎试验测定血浆DAO、LPS。结果治疗前,两组ALB、PA、occludin与s Ig A水平比较差异无统计学意义(P>0.05),治疗后,全部患者上述指标明显高于治疗前,观察组明显高于对照组(P<0.05)。治疗前,两组血浆DAO和LPS水平比较差异无统计学意义(P>0.05),治疗后,全部患者血浆DAO和LPS水平明显低于治疗前,观察组明显低于对照组(P<0.05)。观察组腹胀腹泻发生率明显低于对照组(P<0.05)。结论肠内营养疗法通过明显改善炎症性肠病患者肠黏膜屏障功能指标,以改善营养状况,值得临床推广与应用。
Objective To study the influence of enteral nutrition therapy for inflammatory bowel disease on occludin,diamine oxidase( DAO),lipopolysaccharide( LPS) and secretive immunoglobulin A( s Ig A) levels. Methods 116 cases of IBD patients were selected during January 2013 and July 2016,all were randomly divided into two groups,58 cases in each group. Control group were given immune enteral nutrition,observation group were given ecological immune enteral nutrition. Serum albumin( ALB) and prealbumin( PA) levels were detected by immune turbidimetry,intestinal mucosa occludin protein and intestinal mucosa plasma cells s Ig A express content were detected by immunohistochemical method,plasma DAO,LPS levels were detected by improved chromogenic limulus chloride matrix test. Results Before treatment,there was no statistical difference of the two groups in the comparison of ALB,PA,occludin,s Ig A( P〈0. 05). After treatment,the above indicators of all patients were significantly higher than those before treatment,and observation group were significantly higher than control group( P〈0. 05).Plasma DAO and LPS levels before treatment had no statistical difference between the two groups( P〈0. 05). The above index of all patients after treatment were lower than that before treatment,observation group were significantly lower than control group( P〈0. 05). The abdominal distension diarrhoea incidence rate of observation group was obviously lower than that of control group( P〈0. 05). Conclusion Ecological immune enteral nutrition therapy can improve nutritional status by obviously improving intestinal mucosal barrier function indicators for inflammatory bowel disease,it is worthy of clinical popularization and application.
