摘要
目的:观察三维强化口腔护理联合体位管理在心外科ICU机械通气患者中的应用效果。方法:选择2016年10~12月收治于我院心外科ICU通过口腔气管插管行机械通气患者94例作为研究对象,并随机等分为对照组和试验组,对照组患者以常规口腔擦洗方式行口腔护理;试验组接受三维强化口腔护理联合体位管理干预。比较两组患者口腔细菌菌落数、口腔溃疡及呼吸机相关性肺炎(VAP)发生情况。结果:试验组机械通气入选者干预后口腔细菌菌落计数、口腔溃疡发生率与VAP发生率均明显低于对照组(P<0.05)。结论:采用三维强化口腔护理联合体位管理对心外科ICU机械通气患者实施干预,可提高患者口腔清洁效果,降低口腔溃疡与VAP发生风险。
Objective： To observe the application effect of three-dimensional intensive oral care combined with body position management in patients with cardiac surgery ICU mechanical ventilation. Methods： A total of 94 patients with mechanical ventilation in the cardiac surgery ICU of our hospital from October 2016 to December 2016 were enrolled in this study. The patients were randomly divided into control group and experimental group. The patients in the control group were treated with conventional oral scrubbing line oral care. The experimental group received three-dimensional intensive oral care management combined with postural management. The number of oral bacteria colonies,oral ulcers and ventilator-associated pneumonia（ VAP） were compared between the two groups. Results： The number of oral bacterial colonies,the incidence of oral ulcer and the incidence of VAP were significantly lower in the experimental group than those in the control group after mechanical ventilation（ P〈0. 05）. Conclusion： Intervention of three-dimensional intensive oral care combined with postural management in patients with cardiac surgery ICU mechanical ventilation can improve the oral cleaning effect and reduce the risk of oral ulcer and VAP.
