摘要
目的探讨肾炎舒颗粒与氯沙坦联合治疗对早期糖尿病肾病(DN)患者尿足细胞及炎症因子的影响。方法收集医院收治的早期DN患者85例,随机分为对照组43例和观察组42例,对照组在常规治疗基础上给予氯沙坦治疗,观察组给予肾炎舒颗粒联合氯沙坦治疗。比较两组治疗前后的血清肌酐(Scr)、血浆脂联素(APN)、足尿细胞相关指标以及炎性因子水平的变化。结果治疗后,两组的Scr和APN水平明显改善(P﹤0.05),胶酶相关载脂蛋白(NGAL)、N-乙酰-β-D-葡萄糖苷酶(NAG)及尿白蛋白/肌酐比值(ACR)显著降低,血清白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)水平明显降低,且上述指标的改善幅度均是观察组显著大于对照组(P﹤0.05)。结论肾炎舒颗粒联合氯沙坦治疗早期DN,可有效保护患者肾功能,减轻肾损伤,同时可明显抑制炎性反应。
Objective To explore the effects of Shen Yan Shu Ke Li combined with losartan on urinary podocyte and inflammatory factors in the patients with early diabetic nephropathy（DN）.Methods A total of 85 patients with DN to receive treatment in our hospital were selected and randomly divided in a control group（n=43） and an observation group（n=42）.On the basis of conventional treatment,the control group was treated with losartan and the observation group with Shen Yan Shu Ke Li combined with losartan.The changes in the serum creatinine（Scr）,adiponectin（APN）,urinary podocyte relevant indexes and the levels of inflammatory factors before and after the treatment between the two groups were compared.Results After the treatment,the levels of Scr and APN in the two groups improved significantly（P〈0.05）; neutrophilgelatinase associated lipocalin（NGAL）,N-acetyl-β-D-glucosaminidase（NAG） and urinary albumin/creatinine ratio（ACR） decreased significantly; the levels of interleukin-6（IL-6）,tumor necrosis factor-α（TNF-α）,high-sensitivity C-reactive protein（hs-CRP） decreased significantly; the improvement of the above indexes was significant greater than that in the control group（P〈0.05）.Conclusion Shen Yan Shu Ke Li combined with losartan can effectively protect the renal function of patients and relieve renal injury,and obviously inhibit inflammatory response in the treatment of early DN.
