摘要
目的:了解基层医疗机构医疗风险因素对医疗纠纷的影响,为更好地开展基层医疗机构医疗风险管理工作提供科学依据参考。方法:采用分层抽样选取贵州省9个地(市,州)基层卫生人员共1266人进行问卷调查,采用因子分析法和结构方程模型法研究基层医疗机构医疗风险因素对医疗纠纷的影响。结果:结构方程模型拟合良好,医疗风险因素中医院管理对医疗纠纷的影响最大,其次是内因,制度落实及工作满意度影响较小(其总效用分别为0.4328、0.42、0.2876、0.1596),服务态度、风险意识、医疗质量、风险告知、风险防范措施及纠纷处理方式等医疗风险内因是医疗纠纷的中介变量。结论:提高管理水平,提升工作满意度,严格制度落实,强化医疗服务质量,有效防控基层医疗机构的医疗纠纷。
Objective To understand the influence of medical risk factors on medical disputes in primary health institutions, and provide references for improving the management of medical risks in primary health institutions. Methods 1266 cases of medi- cal staffs were chosen from primary health institutions in nine districts with stratified sampling, and surveyed with questionnaire. The influence of medical risk factors on medical disputes in primary health institutions was studied with factor analysis and struc- tural equation model. Results The model had fine fitting effect, in which the factor of hospital management had the greatest influ- ence on medical dispute, followed with internal factors. And the factors of system implementation and job satisfaction had the low- est effect. The internal factors of service attitude, risk awareness, medical quality, risk notification, risk prevention measures and dispute settlement were mediator variables. Conclusion It is suggested to improve hospital management, promote job satisfaction, intensify system implementation, and strengthen the quality of medical services for preventing and controlling medical disputes.
