外周血Th9和IL-9水平在COPD发病中的作用 被引量:6

The effect of Th9 and IL-9 in peripheral blood of COPD

摘要 目的探讨外周血中Th9比例及IL-9水平在慢性阻塞性肺疾病(COPD)发病过程中的作用及临床意义。方法选择176例COPD患者为研究对象,按照病情不同分为COPD稳定期(n=73)和COPD急性加重期(AECOPD)(n=103),并取健康非吸烟人群85例和健康吸烟人群60例作为对照组。对四组患者进行肺功能测试,采用流式细胞术法(FCM)和酶联免疫吸附试验(ELISA)检测其外周血Th9比例及IL-9水平。结果 COPD稳定期和AECOPD患者第1秒用力呼气容积(FEV_1)、最大呼气流量(PEF)、FEV_1/FVC及6 min步行试验(6MWT)值均显著低于健康非吸烟组及健康吸烟组,差异有统计学意义(P<0.05),AECOPD患者上述指标水平均低于COPD稳定期患者,差异有统计学意义(P<0.05)。AECOPD组患者外周血Th9比例及IL-9水平高于COPD稳定期及健康对照组,差异有统计学意义(P<0.05),COPD稳定期Th9比例及IL-9水平与健康对照组比较也明显提高,差异有统计学意义(P<0.05),健康非吸烟组与健康吸烟组间Th9比例及IL-9水平无明显差别。AECOPD患者IL-9水平与Th9比例呈轻度正相关(r=0.578,P<0.05),COPD稳定期组两者不具有相关性。结论 Th9细胞可能通过分泌IL-9在COPD的疾病进展中发挥重要作用,IL-9可以作为COPD呼吸道炎症的检测指标之一。 Objective The study the clinical significance of Th9 cell and interleukin-9(IL-9)level in peripheral blood of patients with chronic obstructive pulmonary disease(COPD)and acute exacerbation chronic obstructive pulmonary disease(AECOPD). Methods 176 patients with COPD were selected as study subjects,and divided into stable group(73 cases)and acute exacerbation group(AECOPD)(103 cases)according to the different conditions of COPD. At the same time,healthy non-smoking(85 cases)and healthy smoking(60 cases)individuals were selected as the healthy control group.Pulmonary function tests were performed. The ratio of Th9 cell and IL-9 level were detected by flow cytometry(FCM)and enzyme linked immunosorbent assay(ELISA),respectively. Results The values of Forced expiratory volume in one second(FEV1),peak expiratory flow(PEF),FEV1/FVC and six-minutes walk test(6MWT)were significantly lower in stable COPD and AECOPD patients than in healthy non-smoking and healthy smoking groups(P〈0.05). The lung function indicators were significantly lower in AECOPD patients than those of the stable COPD patients(P〈0.05). The ratio of Th9 cell and IL-9 level in AECOPD group were significantly higher than those in stable COPD group and healthy control groups(P〈0.05). The ratio of Th9 cell and IL-9 level in stable COPD group were significantly higher than those in healthy control groups(P〈0.05). There were no significant difference in the ratio of Th9 and IL-9 level between healthy non-smoking group and healthy smoking group. There was a positive correlation between IL-9 level and Th9 ratio in AECOPD patients(r=0.578,P〈0.05),and there was no correlation between the two groups. Conclusions Th9 cell might play an important role in the progression of COPD by secreting IL-9. IL-9 could be one of the indicators in airway inflammation of COPD.
