摘要
目的探讨稀释氧化亚氮对儿童周围静脉穿刺操作性疼痛的干预效果。方法将200名年龄3~14岁需进行周围静脉穿刺操作的非危重症住院患儿随机分为对照组和干预组:对照组患儿在穿刺操作时行常规护理,干预组患儿在穿刺操作前吸入稀释氧化亚氮,气体吸入直至操作完成,其他与对照组相同。分析比较2组患儿的穿刺操作痛评分及穿刺前后收缩压、舒张压、心率和脉搏氧饱和度的变化;统计分析干预组患儿对稀释氧化亚氮吸入镇痛方法的接受度、家属的满意度及患儿吸入后不良反应的发生情况。结果与对照组相比,干预组穿刺操作痛评分明显降低(P<0.001),操作前后血压、心率较平稳(P<0.001);干预组患儿吸入稀释氧化亚氮后无明显不良反应发生,患儿接受度和家属满意度均较高。结论稀释氧化亚氮对儿童周围静脉穿刺操作性疼痛的缓解安全有效。
Objective To assess the analgesia effect of a fixed nitrous oxide/oxygen mixture in the pediatric peripheral veni- puncture procedural pain management. Methods 200 hospitalized children aged 3 to 14 years old with non - critical disease, scheduled to undergo peripheral venipuncture were randomly allocated to either control group or intervention group. Children in control group re- ceived the conventional nursing care and children in intervention group inhaled a fixed nitrous oxide/oxygen mixture before venipuncture, inhaled the gas mixture until venipuncture finished, the conventional nursing care aligned with those in control group. Compared the scores of pain assessment, changes of systolic blood pressure, diastolic blood pressure, heart rate and oxygen saturation before and after venipunc- ture between two groups. The acceptability of children and satisfaction of their parents and negative - effects were also recorded. Results Compared with children in control group, the pain scores were significantly lower among these children in intervention group （P 〈 0.05 ）, and their blood pressure and heart rate showed a significantly smooth trend before and after （P 〈 0.05 ）. There were no ob- vious negative effects in intervention group, children all were willing to use this method again and their parents were all satisfied with the pain remission effect. Conclusion The fixed nitrous oxide/oxygen mixture is efficacious and safe in pediatric peripheral venipuncture procedural pain management.
