摘要
目的观察中药熏洗液对改善混合痔患者术后疼痛的临床效果。方法将医院收治的98例混合痔术后患者随机分为观察组和对照组,每组49例。观察组采用中药煎液(紫草、黄柏、连翘、芒硝等)进行肛门局部熏洗及坐浴,而对照组则同期采用1∶5 000高锰酸钾溶液进行坐浴。利用视觉模拟评分法(VAS)对2组患者术后1 d、5 d疼痛情况进行评分,观察2组患者术后肛门疼痛感消失时间;采用焦虑自评量表(SAS)对患者术后1 d、5 d的焦虑程度进行有效评估;调查患者出院时对其进行满意度。结果 2组患者术后1 d疼痛情况比较差异无统计学意义(P>0.05);2组术后5 d疼痛均有所缓解,观察组术后5 d疼痛评分低于对照组(P<0.01)。观察组术后疼痛消失时间为(3.24±1.03)d,明显短于对照组的(6.22±1.48)d(P<0.01)。术后1 d,2组SAS、SDS评分比较差异无统计学意义(P>0.05);观察组术后5 d SAS、SDS评分均较术后1 d降低,且观察组SAS、SDS评分低于对照组(P均<0.01)。出院时,观察组患者满意度为95.92%,高于对照组的81.63%(P<0.05)。结论中药熏洗液对于改善混合痔患者术后疼痛及疼痛所带来的负性情绪具有确切疗效,值得临床推广应用。
Objective To observe the clinical effect of Chinese medicine fumigation liquid on improving postoperative pain in patients with mixed hemorrhoids. Methods 98 cases of mixed hemorrhoid patients in our hospital were randomly divided into observation group（ n = 49） and control group（ n = 49）. Observation group using traditional Chinese medicine decoction（ Lithospermum,Cork,Forsythia,Glauber＇s salt,etc.） for anal fumigation and bath. While the control group over the same period with 1： 5 000 potassium permanganate solution for bath. The visual acuity（ VAS） was used to evaluate the pain of the two groups at lst day and 5th day after operation. The time of disappearance of anal pain was observed in the two groups. The self-rating anxiety scale（ SAS） D,5 d of the degree of anxiety to conduct an effective assessment of the patient discharged to its satisfaction survey. Results There was no significant difference in pain between the two groups in 1st day after the operation（ P〈0. 05）. Pain was relieved at 5 days after operation and the pain score of the observation group was lower than that of the control group（ P〈0. 01）. The time of pain disappeared was（ 3. 24 ± 1. 03） d in the observation group,which was significantly shorter than that in the control group（ 6. 22 ± 1. 48） d（ P〈0. 01）. There were no significant differences in SAS and SDS scores between the two groups at 1st days after operation（ P〈0. 05）. SAS and SDS scores in the observation group were decreased at 5th day after operation comparing to those at 1st,and SAS,SDS score Lower than the control group（ P〈0. 01）.At the time of discharge,the satisfaction rate of the observation group was 95. 92%,which was higher than that of 81. 63% in control group（ P〈0. 05）.
