摘要
目的:初步探讨胃癌磁共振扩散加权成像(DWI)表观扩散系数(ADC)值与表皮生长因子受体(EGFR)、Ki-67表达的相关性。方法:回顾性分析68例胃癌患者的临床及MRI资料,同时采用免疫组化法检测胃癌中EGFR及Ki-67的表达,分析ADC值(包括平均ADC值和最小ADC值)与EGFR、Ki-67表达的关系。结果:平均ADC值及最小ADC值在不同EGFR表达状态中差异均有统计学意义(P<0.05),其中EGFR表达中阴性组的平均ADC值和最小ADC值均高于EGFR阳性组[(1.106±0.196)×10^(-3) mm^2/s和(0.918±0.164)×10^(-3) mm^2/s,P<0.001;(0.989±0.198)×10^(-3) mm^2/s和(0.835±0.170)×10^(-3) mm^2/s,P=0.002]。EGFR表达与平均ADC值、最小ADC值间均存在显著相关性(r=-0.432,P<0.001;r=-0.382,P=0.001),而Ki-67表达与ADC值间无明显相关性(P>0.05)。结论:胃癌ADC值与肿瘤细胞EGFR表达间存在相关性,ADC值有望作为临床治疗胃癌时辅助评估EGFR表达的指标。
Objective:To preliminarily investigate the correlation of apparent diffusion coefficient(ADC)value acquired from MR diffusion-weighted imaging(MR DWI)with molecular markers(EGFR and Ki-67)in patients with gastric cancer.Methods:The clinical and MRI materials of 68 patients with gastric cancer were analyzed retrospectively,EGFR and Ki-67 expression by immunohistochemistry were acquired simultaneously.Correlation between ADC values,including the mean and minimal ADC value,and EGFR,Ki-67 expression were analyzed,respectively.Results:There were significant differences in ADC values(mean and minimal)with different status of EGFR expression(P〈0.05).In EGFR-negative group,the mean and minimal ADC value were significantly higher than those of EGFR-positive group,respectively[(1.106±0.196)×10^-3 mm^2/s vs(0.918±0.164)×10^-3 mm^2/s,P〈0.001;(0.989±0.198)×10^-3 mm^2/s vs(0.835±0.170)×10^-3 mm^2/s,P= 0.002].The mean and minimal ADC value correlated significantly with EGFR expression,respectively(r=-0.432,P〈0.001;r=-0.382,P=0.001).However,there was no significant difference between Ki-67 expression and ADC value(P〉0.05).Conclusion:Correlation do exist between ADC values and EGFR expression;ADC value might be used as a noninvasive biological marker to evaluate the EGFR expression in clinical treatment of gastric cancer.
