家庭氧疗对慢性阻塞性肺疾病低氧血症肺动脉高压和认知损害的影响 被引量:3

摘要 目的分析慢性阻塞性肺疾病(COPD)长期缺氧导致肺动脉高压和认知损害,予家庭氧疗带来的益处。方法选择78例COPD稳定期患者与健康居民78例作为对照组,测定血氧饱和度、氧分压、肺功能、彩色多普勒心脏超声、简易精神状态法测定认知功能,利用统计学方法测定是否肺功能及肺外系统损害有关,家庭氧疗3月后对上述指标进行复测。结果 COPD合并肺动脉高压(PH组)PaO_2([57.87±12.39)mmHg]明显低于COPD组([66.8mmHg±12.3)mmHg]和对照组([85.45±3.610)mmHg],3组之间差异有统计学意义(P<0.05)且Pa02和肺动脉压力呈负相关。COPD认知功能损害(MMSE)总分明显低于对照组,血氧饱和度降低与认知损害的发生差异有统计学意义(P<0.05)。家庭氧疗提高PaO_2、SP0_2、FEV%/FVC(%),降低MMSF和PAPS(P<0.001)。结论 COPD低氧血症引起肺动脉高压及认知损害的高危因素,损害与低氧血症程度相关,长期氧疗带来的潜在益处,从而延长COPD患者生命及提高生活质量。 Objective To investigate the harm of the chronic obstructive pulmonary disease(COPD) hypoxia to pulmonary hypertension and impaired cognitive function,and discuss about the benefits of home oxygen therapy. Methods 78 cases of COPD patients were chosen and 78 cases of healthy residents were selected as control group at the same time. Blood oxygen saturation,blood gas analysis,pulmonary function,color doppler echocardiogram were determined,and cognitive function was determined through simple mental state method. Statistical analysis was used to determine whether the lung function was correlated with lung damage. And all indexes were determined again after all patients with COPD were given home oxygen therapy for three months. Results PaO_2 mean values in COPD group with pulmonary hypertension(PH group)was 57.87±12.39 mmHg which was lower than that in group COPD(66.8 mmH g±12.3)mmHg)and control group(85.45±3.610 mmHg),and the difference among three groups was statistically significant(P0.05).PaO_2 was negatively correlated with pulmonary artery pressure. Cognitive impairmen(tMMSE)score and the directional force,memory and computing power and attention,memory and language skills in COPD group were lower than those in control group. Lower blood oxygen saturation and the development of cognitive impairment were statistically significantly different(χ~2= 0.22, P0.05). Regular home oxygen therapy could significantly reduce the degree of damage(χ~2= 0.88, P0.001). Conclusion COPD hypoxemia can caused pulmonary hypertension and cognitive impairment,damage degree was associated with the degree of hypoxemia. Long-term oxygen therapy has potential benefits so as to prolong life and improve the quality of life of patients with COPD.
