摘要
目的探讨鼻内镜下低温等离子腺样体切除术的疗效,同时与传统腺样体刮除术比较在术后出血、平均住院日、咽鼓管咽口及圆枕保护、腺样体残留等方面优势。方法选取2010年1月—2016年12月因腺样体肥大在我科行鼻内镜下低温等离子腺样体切除术患儿为研究组,选取2010年1月以前在我科行腺样体刮除术的患儿为对照组。术前所有患儿均经3个月以上保守治疗无效,术后随访6个月。结果鼻内镜下低温等离子腺样体切除对改善患儿鼻腔通气,睡眠打鼾,分泌性中耳炎,副鼻窦炎等方面有重要意义。与传统腺样体刮除术相比,鼻内镜下低温等离子腺样体体切除术在保护咽鼓管咽口及圆枕,腺样体残留方面有明显优势,P<0.05,差异有统计学意义,但在术后出血、平均住院日及术后疼痛等方面对比,P>0.05,差异无统计学意义。结论与传统的腺样体刮除术相比,鼻内镜下低温等离子腺样体体切除具有安全、可靠的优点,能改善患儿因腺样体肥大引起的相关症状,是目前治疗儿童鼾症及分泌性中耳炎的首选方法。
Objective To investigate the effect of endoscopic resection of low temperature plasma adenoidectomy,compared with the traditional adenoids curettage,the postoperative bleeding,average length of stay,protection of eustachian tube,pharyngeal orifice and round pillow,advantages of residual adenoids.Methods From January 2010 to December 2016,the patients with adenoid hypertrophy underwent endoscopic adenoidectomy under low temperature were selected as the study group,children who underwent adenoids curettage in our hospital before January 2010 were selected as the control group.All the children were treated conservatively for more than 3 months before operation,they were followed up for 6 months.Results Nasal endoscopic adenoidectomy under low temperature to improve nasal ventilation in children,sleep snoring,otitis media with secretory otitis media,paranasal sinusitis and other aspects of great significance.Compared with traditional adenoids curettage,endoscopic hypothermia plasma adenoidectomy for the protection of the eustachian tube,pharyngeal orifice and torus,there were obvious advantages in the residual of adenoids,P〈0.05,the difference was statistically significant,but there were no statistically significant differences in postoperative bleeding,average length of stay and postoperative pain,P〈0.05.Conclusion Compared with traditional adenoidectomy,endoscopic resection of low temperature plasma adenoidectomy is safe and reliable,it can improve the children's symptoms caused by adenoidal hypertrophy.It is the first choice for the treatment of snoring and secretory otitis media.
