摘要
目的探讨内镜下金属钛夹联合黏膜下注射白眉蛇毒血凝酶对上消化道出血患者成功止血率及预后的影响。方法选取我院治疗的上消化道出血98例患者,随机数字表法分为两组,各49例。对照组行胃镜下金属钛夹治疗,观察组行内镜下金属钛夹+黏膜下注射白眉蛇毒血凝酶治疗。统计比较两组治疗效果(即时止血率、成功止血率、再出血率)及治疗前后SF-36生活质量调查问卷评分变化。结果观察组即时止血率(95.92%)与对照组(87.76%)比较,差异无统计学意义(P>0.05);观察组成功止血率(91.84%)高于对照组(73.47%),且再出血率(4.08%)低于对照组(24.49%),差异均有统计学意义(P<0.05);治疗后观察组SF-36各项评分均高于对照组,差异有统计学意义(P<0.05)。结论联合内镜下金属钛夹及黏膜下注射白眉蛇毒血凝酶可降低上消化道出血患者再出血率,并提高成功止血率及生活质量,预后效果较好。
Objective To investigate the effect of endoscopic titanium titanium clip combined with submucosal injection of white eyebrow snake venom blood coagulation on the success rate and prognosis of patients with upper gastrointestinal hemorrhage.Methods 98 patients with upper gastrointestinal bleeding treated by our hospital were randomly divided into two groups：49 cases.The rats in the control group underwent endoscopic titanium titanium clip treatment.The observation group was treated with endoscopic metal titanium clip and submucosal injection.The curative effect of SF-36 on the quality of life of the two groups was compared between the two groups before and after treatment（immediate hemostasis rate,success rate of hemostasis and rebleeding rate）.Results The rate of hemostasis（91.84%）was significantly higher in the observation group than in the control group（73.47%）,and the rate of hemostasis was significantly higher in the observation group（95.92%）than that in the control group（87.76%）,and the difference was not statistically significant（P〈0.05）.The scores of SF-36 in the observation group were higher than those in the control group（P〈0.05）,the difference was statistically significant（P〈0.05）,and the difference was statistically significant（P〈0.05）.Conclusion The combination of endoscopic titanium medulla and submucosal injection of white eyebrow venom coagulation enzyme can reduce the rate of rebleeding in patients with upper gastrointestinal bleeding,and improve the success rate of bleeding and quality of life,the prognosis is better.
