摘要
目的探讨Graves甲亢和Graves眼病患者不同治疗方法治疗前后外周血单个核细胞中Th1、Th2、Th17、Treg细胞转录因子TBX21、GATA3、RORC、FOXP3 mRNA表达水平的变化及其意义。方法选择2014年9月至2016年5月就诊于西安交通大学第一附属医院内分泌门诊的新发未治的Graves甲亢患者12例,给予甲硫咪唑治疗(MMI);Graves眼病患者12例给予甲强龙冲击治疗(IVMP),采集患者治疗前后外周血,提取外周血单个核细胞RNA,通过实时荧光定量PCR检测Th1、Th2、Th17及Treg细胞转录因子(TBX21、GATA3、RORC及FOXP3)mRNA的表达情况。结果 Graves甲亢病组治疗缓解后与治疗前比较,RORC[(0.090 2±0.036 4)vs(0.292 6±0.198 6)]和FOXP3[(0.088 9±0.058 7)vs(0.148 6±0.072 6)]的表达均明显增加,差异具有统计学意义(P<0.05);Graves眼病组治疗前后相比TBX21[(0.043 7±0.026 0)vs(0.043 6±0.019 6)]、GATA3[(0.040 5±0.023 3)vs(0.043 7±0.015 8)]、RORC[(0.184 4±0.094 9)vs(0.174 5±0.066 5)]及FOXP3[(0.1324±0.075 7)vs(0.187 0±0.059 5)]差异无统计学意义(P>0.05)。治疗前Graves眼病组与Graves病组比较RORC[(0.184 4±0.094 9)vs(0.090 2±0.036 4)]的表达显著高,且差异有统计学意义(P<0.01)。结论 MMI和IVMP治疗会影响T细胞的分化;各T细胞亚群转录因子mRNA表达水平的变化与疾病的发展和治疗结果密切相关。
Objective To investigate the mRNA expression of Th1, Th2, Th17 and Treg transcription factors(TBX21, GATA3, RORC and FOXP3) in peripheral blood mononuclear cells(PBMCs) of patients with Graves' hyperthyroidism and Graves' ophthalmopathy treated by methimazole(MMI) and methylprednisolone pulse therapy(IVMP),respectively. Methods A to tal of 12 patients with early-onset Graves' hyperthyroidism and 12 patients with Graves' ophthalmopathy, who admitted to Department of Endocrinology of the First Affiliated Hospital of Xi'an Jiaotong University from September 2014 to May 2016, were respectively treated with MMI and IVMP. PBMCs were obtained before and after different the treatment, and the RNAs of PBMCs were extracted. q RT-PCR was used to analyze the mRNA expression levels of Th1, Th2, Th17 and Treg transcription factors(TBX21, GATA3, RORC and FOXP3). Results The mRNA level of RORC and FOXP3 of Graves' hyperthyroidism group before the treatment of MMI were respectively(0.090 2±0.036 4) and (0.088 9±0.058 7), which were significantly lower than(0.292 6±0.198 6) and (0.148 6±0.072 6)after the treatment(P〈0.05). Moreover, there was no significantly differences in the expression of TBX21(0.0437 ±0.0260) vs (0.043 6±0.019 6), GATA3(0.040 5±0.023 3) vs (0.043 7±0.015 8), RORC(0.184 4±0.094 9) vs (0.174 5±0.066 5) and FOXP3(0.132 4 ± 0.075 7) vs (0.187 0 ± 0.059 5) in Graves' ophthalmopathy patients before and after the treatment of IVMP. However, the expression of RORC(0.184 4±0.094 9) vs (0.090 2±0.0364) in Graves' ophthalmopathy group was significantly higher than that in Graves' hyperthyroidism group(P〈0.01). Conclusion MMI and IVMP treatment have effect on the differentiation of T cells. The changes in the mRNA expression of the transcription factors of each T cell subset are closely related to the development of the disease and the outcome of the treatment.
