摘要 目的初筛不同年龄组hrHPV阳性标本,同时行HPV亚型分析及液基细胞学检查,了解银川地区HPV感染基因型分布情况,探讨不同卫生资源条件下的宫颈癌筛查策略。方法选取妇科就诊16~70岁女性1 874名,采用新柏氏液基细胞学技术、HC2及HPV导流杂交分型等方法对宫颈刷出物进行检验,对细胞学异常者召回行阴道镜检查。结果 HC2初筛阳性率为16.21%,液基细胞学异常率为6.1%,HPV 16型为最常见的HPV感染型别,感染率为7.8%,HPV阳性率和HPV 16型阳性率均与细胞学病变级别呈正相关性(r为0.253、0.226,P<0.05)。结论 HPV核酸检测对宫颈癌的筛查效率高于液基细胞学。HPV亚型分析与HC2在宫颈癌筛查敏感性方面差异无统计学意义,但HC2具备操作简便、筛查成本低等优点,不失为为宫颈癌筛查的首选方法,特别是在卫生资源相对欠缺地区。 Objective To screen the hrHPV positive specimen from different age groups, then analyze the subtypes and TCT screening, in order to unveil the HPV distribution of Yinchuan, and search the optimization strategy for cervical cancer screening under the imhalanced medical resource. Methods A prospective clinical study was made and a total of 1874 women aged 16 ~ 70 were select- ed. Cervical brushes were retrospectively analyzed by thinprep cytologic test (TCT), HC2 and HPV hybridization methods. Those women with abnormal cytological results were referred to colposcopy. Results The positivity of HC2 and cytology was 16.21% and 6.1% re- spectively. Subtypel6 was the most common infective HPV types with a positive rate of 7.8%. Both the total HPV infection rate and sub- typel6 infection rate were positively associated with the grade of cytological abnormity with a correlation coefficient of 0.253 and 0. 226 respectively (P 〈 0.05 ). Conclusion This study shows that HPV nucleic acid detection is an effective method than TCT. HPV subtypes analysis and HC2 have approximate sensitivity, hut HC2 is easy to manipulate and has the lower cost. For primary screening cervical cancer,it may be the best choice, especially on the worse medical region.
