摘要
目的探讨医疗失效模式和效应分析(Healthcare Failure Mode and Effect Analysis,HFMEA)在药学部危化品储运管理中的应用效果。方法本院药学部在2015年7月至2016年1月期间,运用HFMEA模式评估药学部危化品储运的流程,从中分析原因,找出每一步骤的潜在失效模式,计算风险优先数值(Risk priority number,RPN)以及优先干预因素,针对高风险因素采取改进措施,使药学部的危化品储运流程得到不断完善。结果应用HFMEA模式后,药学部危化品储运情况明显改进,RPN值由623下降至55,差异有统计学意义(P<0.05);请领周期由无序规范为一周,各部门存储量降低至一周量,发放耗时下降明显。结论 HFMEA模式的运用可以预见药学部危化品储运的失效环节,确定安全重点改进目标,最终通过实践措施,确保安全。
Objective To explore the application and effect of Healthcare Failure Mode and Effect Analysis(HFMEA) in safety management of hazardous chemical substance storage and transportation. Method The HFMEA was applied in department of pharmacy from July 2015 to January 2016, which aimed to assess procedures, calculate risk priority number(RPN) and find out preferred intervention factor. Based on that, we further improved the safety management of hazardous chemical substance storage and transportation. Results After application of the HFMEA, RPN value statistically declined from 623 to 55(P0.05). The quantity of hazardous chemical substance storage in each department declined to one-week usage, and transportation time also declined obviously. Conclusion The HFMEA could be used to predict failures in the procedure of hazardous chemical substance storage and transportation and find out key problems.
