摘要
目的:探讨s PLA2在未足月胎膜早破(PPROM)中的表达意义,以及预测未足月胎膜早破早产的价值。方法:选择2014年2月-2016年3月收治的未足月胎膜早破患者150例作为未足月胎膜早破组,按诊疗规范给予相应治疗,根据治疗转归分为1周内分娩90例,1周后分娩60例。同时期随机选择满28周至不足37周正常产检孕妇45例作为对照组。通过免疫比浊法及ELISA双抗体夹心法测定研究对象入院时的CRP、IL-6及sPLA2含量,并进行比较。结果:未足月胎膜早破组的CRP、IL-6和sPLA2表达均高于对照组,差异均有统计学意义(P<0.01)。1周内分娩的s PLA2表达高于1周后分娩,差异有统计学意义(P<0.01);1周内分娩和1周后分娩的CRP、IL-6比较,差异均无统计学意义(P>0.05)。结论:sPLA2、CRP和IL-6均能动态监测未足月胎膜早破的感染状态;s PLA2还可反映未足月胎膜早破早产临产,从而指导临床保胎及必要时第二疗程促胎肺成熟,做到对早产高危者及时保胎及促胎肺成熟,以提高早产儿存活率,同时对早产低危者避免过度药物治疗。
Objective：To explore the significance of secretory phospholipase A2（s PLA2）in preterm premature rupture of membranes（PPROM）and the value of predicting premature delivery.Method：From February 2014 to March 2016,150 cases of preterm premature rupture of membranes were selected as preterm premature rupture of membranes group,according to the standard of treatment to give appropriate treatment,according to treatment outcomes,they were divided into within 1 week of delivery（n=90） and after 1 week of delivery（n=60）.At the same time,45 patients were randomly selected for 28 weeks to 37 weeks,and the normal pregnant women were selected as the control group.Immunoturbidimetry and ELISA double antibody were used to detect the expression levels of CRP,IL-6 and s PLA2 in admission to hospital.Result：The expression of CRP,IL-6 and s PLA2 in the preterm premature rupture of membranes group were higher than those in the control group,the differences were statistically significant（P〈0.01）.Within 1 week of delivery of s PLA2 was higher than that in 1 week after delivery,the difference was statistically significant（P〈0.01）;CRP,IL-6 of delivery within 1 week and 1 week after delivery comparison,there were no significant differences（P〉0.05）.Conclusion：s PLA2,CRP and IL-6 can dynamically monitor the infection status of preterm premature rupture of membranes;s PLA2 can also reflect the PPRM preterm labor,so as to guide the clinical miscarriage and necessary second course of promoting fetal lung maturation,to do timely high-risk premature miscarriage and promoting fetal lung maturation,in order to improve the survival rate of premature infants,while avoiding excessive drug treatment for premature and low risk.
