摘要
目的探讨结直肠癌初治患者外周血中性粒细胞与淋巴细胞比值(NLR)与临床病理特征及预后的关系。方法选取结直肠癌患者387例,观察患者术前NLR与临床病理特征及预后的关系。结果不同性别、年龄、肿瘤部位、肿瘤大小、淋巴结转移、远处转移、浸润程度和分化程度结直肠癌患者的NLR比较,差异无统计学意义(P﹥0.05);血小板≥350×10~9/L结直肠癌患者的术前NLR高于血小板﹤350×10~9/L的患者(P﹤0.05);TNM分期为Ⅲ、Ⅱ期结直肠癌患者的术前NLR均高于Ⅰ期患者(P﹤0.05);高NLR组患者的中位生存时间为35.41个月(95%CI:25.93~46.03),低于低NLR组的42.81个月(95%CI:33.39~58.10)(P﹤0.05);低NLR组TNM分期为Ⅰ、Ⅱ、Ⅲ期患者的中位生存期分别为42.65个月、42.01个月和41.04个月,差异无统计学意义(χ~2=2.382,P﹥0.05);高NLR组TNM分期为Ⅰ、Ⅱ、Ⅲ期患者的中位生存期分别为37.03个月、36.52个月和35.02个月,差异无统计学意义(χ~2=3.082,P﹥0.05)。结论结直肠癌初治患者术前NLR与患者的临床病理特征及预后有一定关系,值得进一步研究。
Objective To investigate the correlation between the neutrophil to lymphocyte ratio（NLR） in peripheral blood and the clinical pathology in the patients with primary colorectal cancer and the prognostic significance. Method387 patients with colorectal cancer were enrolled in the study, of which the preoperative NLR and clinical pathology were observed and investigated. Result Preoperative NLR was independent of gender, age, tumor location, tumor size, lymph node metastasis, distant metastasis, invasion and differentiation（P〈0.05）; patients with platelet count ≥ 350 × 10^9/L had higher preoperative NLR than those with platelet 350 × 10^9/L（P〈0.05）; the preoperative NLR in patients with TNM stage Ⅱ or Ⅲ were higher than those with stage I（P〈0.05）; high NLR group had a median survival time of 35.41 months（95%CI： 25.93-46.03）, which was significantly lower than that in low NLR group at 42.81 months（95%CI： 33.39-58.10）（P〈0.05）; the median survival time of patients with TNM Ⅰ, Ⅱ and Ⅲ in low NLR group were 42.65, 42.01 and 41.04 months, respectively, with no significant difference observed（χ2=2.382, P〈0.05）; and that in high NLR group were 37.03,36.52 and 35.02 months, respectively, and the difference was not statistically significant（χ2=3.082, P〈0.05）. Conclusion The preoperative NLR in patients with colorectal cancer is associated with the clinical pathology and prognosis of patients with colorectal cancer, further study is warranted.
