摘要
目的探讨内镜下套扎和组织黏合剂法对食管胃底静脉曲张出血的治疗作用及对门脉血流动力学的影响。方法94例食管胃底静脉曲张患者按照治疗方法分为对照组（n=47）和观察组（n=47）。对照组采用组织黏合剂法，观察组采用内镜下套扎法，比较两组临床效果、检测对门脉血流动力学的影响，并记录并发症发生情况。结果两组患者止血成功率、早期再出血率、迟发性再出血率、静脉曲张改善总有效率及病死率比较差异未见统计学意义（P〉0．05）。对照组血流动力学指标比较差异未见统计学意义（P〉0．05）；观察组治疗后门静脉和脾静脉血流速度有所提高，但差异未见统计学意义（P〉0．05）。两组患者的并发症主要为发热、胸痛、腹胀、恶心呕吐、进食哽咽感、食管腔狭窄，两组并发症发生率比较差异未见统计学意义（P〉0．05）。结论组织黏合剂和内镜下静脉套扎法对食管胃底静脉曲张患者的临床效果相当，对血流动力学无明显影响，但两种不同方法具有各自优点，临床治疗过程中应根据患者实际情况进行选择。
Objective To investigate the effects of endoscopic ligation and tissue adhesives on endoscopic variceal and portal hemodynamics. Methods Ninety-four patients with endoscopic variceal were divided into control group （n = 47） and observation group （n = 47 ）. The patients in control group were given tissue adhesives, and the patients in observation group were given endoscopic ligation. The effects, portal hemodynamics and compilations were compared. Results The success rate of hemostasis, early re-bleeding, rehaemorrhage ratio, endoscopic variceal improvement rate and mortality between the two groups had no significant difference （ P 〉 0. 05 ）. The blood flow velocity of portal vein and splenic vein in the observation group after treatment was improved, but there was no significant difference （P 〉 0.05 ）. The incidence of complications after surgery between the two groups had no significanty difference （P 〉 0. 05 ）. Conclusions The endoscopic ligation and tissue adhesives can be used in treatment of en- doscopic variceal, and has no influence on hemodynamic parameters. The two methods both have their advantages, it is necessary to choose proper ways arrcording to the patient＇ s actual situation in the clinical treatment.
