18F-FDG PET/CT Features of Cutaneous Lymphoma

摘要 目的 分析初治前皮肤淋巴瘤的18F-FDG PET/CT显像特点.方法 回顾性分析行18F-FDG PET/CT检查的初治前皮肤淋巴瘤患者,共计17例,年龄12~82岁,中位年龄42岁;男8例,女9例;T/NK细胞淋巴瘤12例,B细胞淋巴瘤5例.采用美国GE Discovery STE 16 PET/CT仪,由2名有经验的核医学医师共同阅片,观察CT所示病变部位的大小、形态、与周围组织关系,以及相应部位PET代谢情况,由XELERIS自动生成最大标准摄取值(SUVmax).结果 1.临床表现:初发皮肤病变部位包括头面部、四肢、躯干部等,皮损表现有局部肿胀者9例,有皮温增高者4例,有表面破溃者3例,有局部疼痛者6例,有局部瘙痒者3例.2.18F-FDG PET/CT征象:皮肤病灶可单发或多发;16例患者的全身多处皮损或皮下结节的PET显像皆可见异常增高的放射性摄取;另1例患者仅有部分皮肤病变伴葡萄糖代谢轻度增高.上述患者除了皮肤受累,另有不同部位淋巴结肿大者14例,骨骼受累者4例,胰腺受累者2例,肝脏、乳腺、脾脏、胃壁、肾脏、胸膜及腹膜受累者各1例.皮肤病灶的CT表现:形态可为絮状、片状、结节状,大多边界不清,皆无钙化,7例有皮肤增厚,5例有皮下组织网格状改变.结论 皮肤淋巴瘤的18F-FDG PET/CT显像特点多样化,18F-FDG PET/CT显像对皮肤淋巴瘤有较高的诊断价值. Objective To analyze the 18F-FDG PET/CT features of cutaneous lymphoma. Methods A retrospective study was performed on 17 untreated cutaneous lymphoma patients who underwent 18F-FDG PET/CT. There were 8 men and 9 women (age range 12~82 years). We separately analyzed the 18F-FDG PET features and CT features of cutaneous lymphoma. Results (1)Clinical manifestation: There are different skin lesion sites including face, chest, back, arm and leg. Different patients suffer from different clinical manifestation such as local swelling, surface rupture, local pain or local itching. (2)PET/CT features: Skin lesion sites can be single or multiple. All skin lesion sites of 16 patients represent high radioactive uptake. Only partial lesions represent high SUV (maximum standardized uptake value) in 1 patient. CT features of skin lesion sites can be nodular, sheet or flocculent. Most lesions have obscure boundary. Some lesions accompany by local skin thickening. Conclusion 18F-FDG PET/CT features of cutaneous lymphoma are diversified. 18F-FDG PET/CT has potential value for diagnosis of cutaneous lymphoma.
