摘要 目的 :探讨心理引导联合自护技能培养对结核病患者生活质量的影响。方法:选取我院2013年8月~2016年8月收治的结核病患者280例,并随机等分为试验组和对照组。入院后均进行规范化抗结核治疗,对照组给予常规护理,试验组在对照组基础上给予心理引导联合自护技能培养干预。分别采用自护能力测定量表(ESCA)、服药依从性量表(MMAS 8-item version)、肺结核患者生存质量测定量表(QLICD-PT)评定两组自我管理能力、服药依从性、生存质量。结果:试验组ESCA评分中健康知识水平、自护责任感、自我护理技能、自我概念评分均明显高于对照组,MMAS 8-item version评分高于对照组(P<0.05)。试验组QLICD-PT评分中消极心理、治疗心理、歧视感、疾病生理、治疗经济、一般生理、社会支持和健康教育各项评分均明显高于对照组(P<0.05)。试验组临床症状消失或显著缓解时间明显较对照组短(P<0.05)。结论:对结核病患者实行心理引导联合自护技能培养干预,可有效提高其治疗依从性、自我管理能力及生活质量。 Objective: To explore the effect of psychological guidance and self-care skill training on quality of life of tuberculosis patients. Methods: A total of 280 tuberculosis patients admitted in our hospital from August 2013 to August 2016 were selected and were randomly divided into experimental group and control group. After the admission they were treated with standardized anti-TB treatment. The control group was given conventional care,while the experimental group was given psychological guidance combined with self-care skills training intervention on the basis of the control group. The self-management ability,medication compliance and quality of life were assessed by self-care ability measurement scale( ESCA),medication compliance scale( MMAS 8-item version) and quality of life instruments for chronic diseases-pulmonary tuberculosis( QLICD-PT). Results: The level of health knowledge,self-care responsibility,self-care skill and self-concept score of ESCA score in the experimental group were significantly higher than those in the control group,and the score of MMAS 8-item version was higher than that of the control group( P < 0. 05). The scores of negative psychology,treatment psychology,discrimination,disease physiology,treatment economy,general physiology,social support and health education were significantly higher in the QLICD-PT score of experimental group than that of the control group( P < 0. 05). The clinical symptoms disappeared or the significant remission time was shorter in the experimental group than in the control group( P < 0. 05). Conclusion: It is effective to improve the treatment compliance,self-management ability and quality of life by the intervention of psychological guidance and self-care skills training for tuberculosis patients.
